Rb、β及γ-catenin蛋白在进展期胃癌及转移淋巴结癌细胞中的表达及临床意义

摘　　要：目的探讨Rb、β与γ-catenin在进展期胃癌及转移淋巴结癌细胞再表达情况及其与临床病理诸因素的关系。方法应用免疫组织化学染色方法检测Rb在肿瘤原发灶及β与γ-catenin在86例进展期胃癌原发灶及其转移淋巴结癌细胞的表达情况。结果胃癌原发灶癌细胞中65.12%出现Rb阳性,38.37%转移淋巴结癌细胞出现β与γ-catenin再表达阳性,Rb与肿瘤分化程度密切相关(P<0.05),β与γ-catenin在转移淋巴结癌细胞再表达阳性与肿瘤分化程度密切相关(P<0.05),γ-catenin在转移淋巴结癌细胞再表达与肿瘤病理类型密切相关(P<0.05),β-catenin在转移淋巴结癌细胞再表达与肿瘤病理类型无关(P>0.05),Rb表达与β-catenin在转移淋巴结癌细胞中再表达情况相关,与肿瘤病理类型、大小、浸润程度等因素无关(P>0.05)。γ与β-catenin在转移淋巴结癌细胞再表达相关(P<0.05),Rb在肿瘤原发灶癌细胞表达和β与γ-catenin在转移淋巴结癌细胞再表达无关(P>0.05)。β与γ-catenin再表达阳性者3年生存率与再表达阴性者比较差异有统计学意义(P<0.05)。结论 Rb表达与进展期胃癌发展过程密切相关,与β-catenin在转移淋巴结癌细胞再表达有关,β与γ-catenin在转移淋巴结癌细胞再表达是进展期胃癌发生过程中的重要事件,二者协同发挥作用,并对癌细胞在转移灶中聚集生长起重要作用,联合评价其功能具有重要临床意义。Objective To investigate the expression of Rb and re-expression of β and γ-catenin in cancerous cells of metastatic lymph nodes from advanced gastric carcinoma. Methods The expression levels of Rb and β and γ-catenin were detected by immunohistochemistry in 86 cases of primary focus of advanced gastric carcinoma as well as corresponding metastasis lymph nodes. Results Among 86 cases of primary focus of advanced gastric carcinoma, the expression of Rb was detected in 56 case （65. 12% ） ,and re-expression of p-and γ-catenin in metastatic cancerous cells was detected in 33 case （38. 37% ）. The expression of Rb was closely related with tumor differentiation degree （P 〈0. 05） ,and the re-expression of p-and γ-catenin was closely correlated to tumor differentiation degree （ P 〈 0. 05 ） . The re-expression of γ-catenin in metastatic cancerous cells was closely correlated to tumor pathological types （P 〈 0. 05 ） , however, the re-expression of pcatenin in metastatic lymph nodes was not related with tumor pathological types （P 〉 0. 05 ） . Moreover the expression of Rb in metastatic lymph nodes was related with that of p-catenin, however, which was not correlated to tumor pathological types, tumor size, infiltration degree （ P 〉 0. 05 ）. There were significant differences in 3 - year survival rates between patients with positive re-expression of β or γ-catenin and patients with negative re-expression of β or γ-catenin. Conclusion The expression of Rb protein is closely related with the progress of advanced gastric carcinoma and with the re-expression of p-catenin in cancerous cells of metastatic lymph nodes. The re-expression of p-and γ-catenin in cancerous cells of metastatic lymph nodes from advanced gastric carcinoma plays an important role in the pathogenesis of advanced gastric carcinoma and both factors have synergistic reaction and play important role in aggregation and growth of cancer cells in metastatic
