摘 要:目的:探讨IMP3基因在葡萄胎中的表达及其意义。方法:应用免疫组织化学链霉亲和素-过氧化物酶(SP)法检测IMP3基因在不同转归患者初次清宫葡萄胎组织中的表达。结果:IMP3基因在人绒毛膜促性腺激素(HCG)>2×10~5 mIU/ml患者和HCG<2×10~5 mIU/ml患者中均有表达;IMP3基因在HCG>2×10~5 mIU/ml葡萄胎患者中阳性表达率为90%,在HCG<2×10~5 mIU/ml患者中的表达率为78%,两者比较差异有统计学意义(x^2=2.245,P<0.05)。在HCG>2×105m IU/ml的葡萄胎患者中,后来发展为侵蚀性葡萄胎患者的IMP3基因表达阳性率高,组间比较差异有统计学意义(x^2=10.719,P<0.05);HCG<2×10~5 mIU/ml葡萄胎患者中,后来发展为侵蚀性葡萄胎患者的IMP3基因表达阳性率高,组间比较差异有统计学意义(x^2=14.438,P<0.05)。结论:IMP3基因作为一种特异性很高的胞浆标志物,表达于葡萄胎合体滋养细胞,在HCG>2×10~5 mIU/ml的葡萄胎组织中表达率高。IMP3基因与葡萄胎的转归密切相关,有望成为判断葡萄胎的发生、发展及转归的标志物。Objective To investigate the expression and significance of IMP3 inhydatidiform mole. Methods: Streptavidin-peroxidase (SP) of immunohistochemistrywas applied to detect expression of IMP3 gene in hydatidiform mole tissue of differentoutcome patients after the first cleaned of their uterus. Results: The IMP3 gene could expressed whether HCGwas higher than 2X105mIU/mL (the positive expression rate was 90%) or lower than 2×10^5mIU/mL ( thepositive expression rate was 78%) in patients, and the difference of the positive expression rate was statisticallysignificant (x2=2.245, P〈0.05). In the patients with hydatidiform mole whose HCG was more than 2×10^5mIU/mL, the positive expression rate of IMP3 gene was higher in a part of patients who developed into invasivehydatidiform mole, and the difference among different groups was statistically significant (x2=10.719, P〈0.05).In the patients with hydatidiform mole whose HCG was less than 2×10^5mIU/mL, the positive expression rateof IMP3 gene also was higher in a part of patients who developed into invasive hydatidiform mole, and thedifference among different groups also was statistically significant (x2=14.438, P〈0.05). Conclusion: IMP3gene which is a higher specificity biomarker in endochylema and it can be expressed in syncytiotrophoblast ofhydatidiform mole. The expression rate of IMP3 gene is higher in the hydatidiform mole tissue which HCG washigher than 2×10^5mIU/mL. There is a closed relationship between IMP3 gene and the outcome of hydatidiformmole, and it may become a biomarker for determining the happen, development and outcome of hydatidiformmole.
