血清表皮生长因子水平与帕金森病患者认知障碍的关系研究

摘　　要：目的探讨血清表皮生长因子(EGF)水平与帕金森病(PD)患者认知障碍的关系。方法选取2015年3月—2017年3月德阳市人民医院收治的PD患者84例作为观察组,根据Hoehn-Yahr(H-Y)分级量表分为早期组52例、中晚期组32例;根据蒙特利尔认知评估量表(MoCA)分为认知障碍组53例与非认知障碍组31例。另选取同期于德阳市人民医院体检健康者40例作为对照组。分别比较观察组与对照组、早期组与中晚期组受试者血清EGF水平、血清脑源性神经营养因子(BDNF)水平、MoCA评分,比较认知障碍组与非认知障碍组患者血清EGF水平、血清BDNF水平、统一帕金森病评定量表(UPDRS)-Ⅲ评分;血清EGF水平与PD并认知障碍患者血清BDNF水平、MoCA评分的相关性分析采用Pearson相关性分析。结果观察组患者血清EGF、BDNF水平和MoCA评分低于对照组(P<0.05)。中晚期组患者血清EGF、BDNF水平和MoCA评分低于早期组(P<0.05)。认知障碍组患者血清EGF、BDNF水平低于非认知障碍组(P<0.05);两组患者UPDRS-Ⅲ评分比较,差异无统计学意义(P>0.05)。Pearson相关性分析结果显示,血清EGF水平与PD并认知障碍患者血清BDNF水平、MoCA评分呈正相关(r值分别为0.754、0.864,P<0.05)。结论血清EGF水平与PD患者认知障碍有关。Objective To explore the relationship between serum epidermal growth factor（ EGF） level and cognitive disorder in patients with Parkinson＇s disease. Methods From March 2015 to March 2017 in the People＇s Hospital of Deyang,a total of 84 patients with Parkinson＇s disease were selected as observation group,and they were furtherly divided into A1 group（ during early stage,n = 52） and A2 group（ during middle and advanced stage,n = 32） according to H-Y Grading Scale,into B1 group（ complicated with cognitive disorder,n = 53） and B2 group（ did not complicated with cognitive disorder,n = 31）according to MoCA; meanwhile a total of 40 healthy people admitted to this hospital for physical examination were selected as control group. Serum levels of EGF and BDNF,and MoCA score were compared between control group and observation group,between A1 group and A2 group,meanwhile serum levels of EGF and BDNF,and UPDRS-Ⅲ score were compared between B1 group and B2 group; Pearson correlation analysis was used to analyze the correlations of serum EGF level with serum BDNF level and MoCA score in Parkinson＇s disease patients complicated with cognitive disorder. Results Serum levels of EGF and BDNF,and MoCA score of observation group were statistically significantly lower than those of control group（ P〈0. 05）. Serum levels of EGF and BDNF,and MoCA score of A2 group were statistically significantly lower than those of A1 group（ P〈0. 05）. Serum levels of EGF and BDNF of B1 group were statistically significantly lower than those of B2 group（ P〈0. 05）, while no statistically significant differences of UPDRS-Ⅲ score was found between B1 group and B2 group（ P〉0. 05）. Pearson correlation analysis results showed,serum EGF level was positively correlated with serum BDNF level（ r = 0. 754） and MoCA score（ r = 0. 864） in Parkinson＇s disease patients complicated with cognitive disorder（ P〈0. 05）.
