补肾活血通络方剂对老年帕金森病痴呆患者认知功能、生活质量及脑脊液IL-6、IL-1β、TNF-α、IFN-γ水平的影响

摘　　要：目的观察补肾活血通络方剂对老年帕金森病痴呆(PDD)患者认知功能、生活质量及脑脊液IL-6、IL-1β、TNF-α、IFN-γ水平的影响。方法将126例老年PDD患者随机分为观察组63例和对照组63例,对照组给予常规西医治疗,观察组在对照组基础上给予补肾活血通络方剂治疗,疗程3个月。观察2组治疗前后认知功能、生活满意度及生活质量情况及脑脊液IL-6、IL-1β、TNF-α、IFN-γ水平。结果 2组治疗后简易精神状态检查表(MMSE)评分中定向力、延迟回忆力、语言能力、即时记忆力、注意力和计算力及总分均有显著提高,生活满意度量表评分(LSIB)、帕金森病生活质量评分量表(PDQ)评分均有显著改善,脑脊液IL-6、IL-1β、TNF-α、IFN-γ水平均有下降,观察组上述指标改善情况均明显优于对照组(P均<0.05);2组治疗期间均未发现严重药物不良事件。结论补肾活血通络方剂能够显著改善老年PDD患者的认知功能及生活质量,提高生活满意度,且安全可靠,其机制可能与抑制机体IL-6、IL-1β、TNF-α、IFN-γ水平有关。Objective： It is to observe the effect of Bushen Huoxue Tongluo decoction on cognitive function, life quality and IL-6, IL-1β,TNF-α, IFN-γ in cerebrospinal fluid in in patients with senile dementia and Parkinson＇ s disease. Methods 126 cases of elderly patients with dementia and Parkinson＇ s disease were randomly divided into observation group （n = 63 ） and control group （n = 63）, the control group was given conventional western medicine treatment, the observation group was treated with Bushen Huoxue Tongluo decoction on the basis of treatment in the control group, both treatment courses were 3 months. The cognitive function, life satisfaction, life quality and the levels of IL-6,IL-1β,TNF-α, IFN-γ in cerebrospinal fluid were observed before and after treatment in the two groups. Results After treatment, the scores of orientation, delayed mem- ory, language ability, immediate memory, attention,calculation and total scores in MMSE were significantly increased, the scores of LSIB and PDQ were significantly improved, and the levels of IL-6, IL-1β,TNF-α, IFN-γin cerebrospinal fluid were obviously decreased, and the improvements of these indexes in observation group were better than those in control group （P 〈0. 05）. No severe side effect was found during treatment in both groups. Conclusion Bushen Huoxue Tongluo decoction could significantly improve the cognitive function, life quality and life satisfaction with good safety and reliability, its mechanism may be related with inhibition of IL-6, IL-1β,TNF-α, IFN-γ in body.
