咀嚼口香糖对根治性膀胱全切+回肠膀胱术后患者胃肠功能恢复的影响

摘　　要：目的:探讨咀嚼口香糖对回肠膀胱术后患者胃肠功能恢复的影响。方法:2014年2月至2015年2月我科对146例恶性膀胱肿瘤患者均实施了根治性膀胱全切+回肠膀胱术,将146例患者随机分为实验组和对照组,每组各73例。对照组采用常规的床上活动的护理方法来促进胃肠功能恢复,实验组在采用床上活动的护理常规的基础上,在患者手术完毕回到病房后6小时后开始咀嚼口香糖,每天早、中、晚3次,每次2片,每次时间15分钟,直到肛门排气。观察两组患者术后首次排气时间以及术后发生腹胀的情况。结果:两组患者术后排气时间差异比较有统计学意义(Z=-4.633,P<0.001),实验组患者术后排气时间短于对照组。结论:咀嚼口香糖有助于根治性全膀胱切除+回肠膀胱术后患者胃肠功能的恢复。咀嚼口香糖是一种价廉、方便的促进肠道蠕动、恢复肠道功能的方法,可作为术后辅助回肠膀胱术患者康复的护理措施。Objective：To explore the effects of chewing gums on gastrointestinal function recovery of patients un- dergoing orthotopic ileal neobladder. Methods ：146 malignant bladder tumor patients, conducted the radical eystecto- my with orthotopic ileal neobladder in our hospital from February 2014 to February 2015 ,were enrolled in our study. All the patients were randomly assigned into two groups ： the test group and the control group with 73 patients respec- tively. The control group received routine bed activities nursing care. On the contrary ,the test group ,besides the above nursing care,began to chew 2 gums each time for 15 minutes 3 times a day until anal passage of gas or defecation. The situation of postoperative abdominal distension and the first time of anal exhaust were compared. Results：The time of first anal exhaust in the test group was shorter than that in the control group, which the difference showed statistical significance （ Z = - 4. 633, P 〈 0. 001 ）. Conclusion ： Chewing gums could effectively promote the gastrointestinal function recovery of patients undergoing orthotopic ileal neobladder. As a cheap and convenient way to improve the gas- trointestinal function,to chew gums is an effective nursing measure to help the rehabilitation of patients after operation.
