神经内科住院患者医院感染病例分析及预防措施

摘　　要：目的分析我院神经内科住院患者医院感染的危险因素,探讨预防与控制医院感染的有效措施。方法 2016年1月至2016年12月在我院神经内科住院患者中74例医院感染病例,与同期住院的患者医院感染总发生率、感染发生部位、年龄、住院天数、侵袭性操作等进行比较。结果神经内科住院患者医院感染发生率高于同期全院医院感染率(χ~2=84.717,P=0.000);下呼吸道感染32例,占感染患者43.24%;不同年龄及不同住院时间的医院感染发生率比较,差异有统计学意义(P<0.05);有侵袭操作发生医院感染的比例明显高于无侵袭性操作(χ~2=11.019,P<0.05)。结论神经内科是医院感染高发科室,对年老、住院时间长、有侵袭性操作的患者应重点关注,应加强病房的基础清洁卫生、通风换气,尽量缩短住院时间,严格执行诊疗操作规程及手卫生制度等环节质量管理。Objective To analyze the risk factors of nosocomial infection at neurology department of our hospital,and to ex- plore effective measures to prevent and control nosocomial infection. Methods From January 2016 to December 2016,there were 74 cases of nosocomial infection at the neurology department. They were compared with the inpatients in the same period for the total inci- dence of nosocomial infection,location of infection, age,length of stay and invasive operation. Results The incidence of nosocomial in- fection in hospitalized patients at the neurology department was higher than that in whole the hospital （X2 = 84. 717 ,P = 0. 000）. Lower respiratory tract infection occurred in 32 cases, accounting for 43.24% of the infected patients. There were significant differences in the incidence of nosocomial infections between different ages and different hospital stay time （ P 〈 0. 05 ）. The rate of nosocomial infection in invasive operation was significantly higher than that without invasive operation （X2 = 11. 019, P 〈 0. 05 ）. Conclusion The depart- merit of neurology is a high incidence department of nosocomial infection. Attention should be paid to the patients who are elderly,long hospital stay and with invasive operations. We should strengthen the basic sanitation and ventilation of the wards, shorten the length of hospital stay, and strictly implement the procedures of operation and treatment as well as the system of hand hygiene.
