血清胃蛋白酶原 胃泌素-17幽门螺杆菌IgG抗体联合检测在胃癌诊断中的临床价值

摘　　要：目的探讨血清胃蛋白酶原（PGI和PGII）、胃泌素-17（G—17）、幽门螺杆菌（Hp）IgG抗体联合检测在慢性萎缩性胃炎和早期胃癌中的临床价值。方法选取2016g-3月至8月就诊的146例胃部疾病患者，根据胃镜检查及组织病理学结果分为三组：慢性萎缩性胃炎组112例．胃癌组34例，对照组66例（包括轻度非萎缩性胃炎）；采用酶联免疫法（ELISA）定量检测受检者空腹血清PGI、PGII和G-17水平，并计算PGI／PGI比值（PGR），定性分析血清Hp-IgG，并进行相关性分析。结果与对照组比较，慢性萎缩性胃炎组中PGI，PGR和G-17水平均降低，差异具有统计学意义（P〈0．05）；胃癌组中PGI和PGR降低，G-17增高，差异具有统计学意义（P〈0．05）；与慢性萎缩性胃炎组比较，胃癌组血清中PGI和PGR值降低，G-17增高，差异均有统计学意义（P〈O．05）。三组Hp—IgG抗体阳性率分别为86．6％、88．2％、53．0％。结论血清PGI、PGR、G-17、Hp．IgG抗体联合检测对萎缩性胃炎和早期胃癌诊断具有一定的临床价值。Objective To investigate the serum pepsinogen, gastrin-17 and Helicobacter pylori IgG antibody in chronic atrophic gastritis and the clinical value of early gastric carcinoma. Methods From April 2016 to August 2016, 146 cases of patients with gastric diseases in our hospital were divided into three groups according to gastroscopy and pathology, 112 cases of chronic atrophic gastritis, 34 cases of gastric cancer, 66 cases of the control group （ including mild non atrophic gastritis ） . Determination by enzyme linked immune method （ ELISA ） quantitative detection of subjects fasting serum pepsinogen （ PGI, PGII ） and gastrin-17 （ G-17 ） level, and the PGI/PGII ratio （ PGR ） was calculated. Serum Helicobacter pylori antibody （ Hp-IgG antibody ） and qualitative were analyzed by correlation analysis. Results Compared with the control group, the PGI, PGR and G-17 were decreased in the chronic atrophic gastritis group （ P〈0.05 ） . The PGI and PGR in gastric carcinoma patients was decreased, G-17 was increased, the difference was statistically significant （ P〈0.05 ） . Compared with chronic atrophic gastritis, the value of PGI and PGR in serum of gastric carcinoma patients was decreased, G-17 was increased, the difference was statistically significant （ P〈0.05 ） . The 3 groups of Hp-lgG antibody positive rate were 86.6%, 88.2%, 53.0%. Conclusion The combined detection of serum PGI, PGR, G-17, Hp-IgG have a certain clinical value in the diagnosis of atrophic gastritis and early gastric cancer.
