人工晶状体脱位手术治疗的临床观察

摘　　要：目的 观察手术治疗人工晶状体（IOL）脱位于玻璃体的安全性与有效性.方法 搜集因后房型IOL脱位于玻璃体接受手术治疗的42例（42只眼）的临床资料进行回顾性分析.手术前单眼复视16只眼,眩光10只眼.术后随访1～3年.观察比较手术效果及并发症发生情况.结果 末次随访时,IOL稳定,未发生IOL偏位或再脱位.术后视力均较术前提高;单眼复视及眩光症状均消失.术后并发症：角膜水肿16只眼,前房葡萄膜炎反应10只眼,高眼压8只眼,低眼压4只眼,前房少量积血4只眼,经药物治疗后均恢复正常.无角膜内皮功能失代偿、继发性青光眼、脉络膜脱离或视网膜脱离等并发症发生.结论 手术治疗后房型IOL脱位于玻璃体安全有效,但应注意手术技巧.Objective To observe the safety and efficacy of vitrectomy for the dislocation of IOL.Methods Forty-two eyes of 42 cases with posterior chamber IOL dislocation into vitreous cavity were collected and retrospectively analyzed.They were treated with vitrectomy.Before surgery,diplopia occurred in 16 eyes.Dazzling sensation happened in 10 eyes.The follow up time was 1-3 years.The surgical efficacy and complications were observed and compared.Results At the end of follow-up,the IOLs were stabilized in the center of pupils.The visual acuity of all cases after surgery were improved.Diplopia and dazzling sensation disappeared.The corneal edema occurred in 16 eyes,the anterior chamber uveitis in 10 eyes,high intraocular pressure in 8 eyes,ocular hypotension in 4 eyes and mild hyphema in 4 eyes.No corneal endothelial decompensation,secondary glaucoma,choroidal detachment or retinal detachment happend.Conclusion Vitrectomy for dislocation into vitreous cavity of PC-IOL is safe and effective.Surgical skills are important to the operative success.
