高度近视硅油填充手术后眼轴长度测量值变化的研究

摘　　要：目的 分析高度近视伴视网膜脱离硅油填充手术后眼轴长度测量值的变化,为确定人工晶状体（IOL）度数提供参考.方法 收集24例（27眼）高度近视伴裂孔源性视网膜脱离需行硅油填充的临床资料,所有患者硅油填充前以IOLMaster进行IOL度数测量,硅油填充后3个月以上,再次用IOLMaster进行IOL度数测量.两次测量均记录眼轴长度、信噪比和中央前房深度,并进行统计分析.结果 患者眼轴长度测量结果：硅油填充前为（28.715±0.862）mm,硅油填充后为（29.338±0.727）mm,差异有统计学意义（t=-2.676,P=0.028）.组合信号信噪比：硅油填充前为92.445±30.815,硅油填充后为81.510±27.170,差异有统计学意义（t=-2.533,P=0.030）.中央前房深度：硅油填充前为（3.575±0.166）μm,硅油填充后为（3.621±0.446）μm,差异无统计学意义（t=-0.096,P=0.928）.结论 高度近视硅油填充后,眼轴长度测量结果较实际长度变长,如需行白内障摘出联合IOL植入术时,有必要以硅油填充前IOLMaster测量结果进行校正.Objective To analyse the measurements changes of axial length after silicone oil tamponade for high myopia and retinal detachment,and to provide a reference for determining the degrees of IOL in high myopia patients.Methods The data of 27 eyes of 24 cases of high myopia with rhegmatogenous retinal detachment were collected.They underwent silicone oil tamponade.The degrees of IOL were measured by IOLMaster before and more than 3 months after silicone oil tamponade.The axial length,signal noise ratio（SNR）and anterior chamber distance were recorded and analyzed.Results Pre-and postoperative average AL were（28.715±0.862）mm and（29.338±0.727）mm.The difference was statistically significant（t=-2.676,P=0.028）.Pre-and postoperative SNR were 92.445±30.815 and 81.510±27.170.The difference was statistically significant（t=-2.533,P=0.030）.Pre-and postoperative ACD were（3.575±0.166）μm and（3.621±0.446）μm.The difference was not significant（t=-0.096,P=0.928）.Conclusion The measurements of the axial length of high myopia eyes after silicone oil tamponade is longer than that before surgery.If the patients receive cataract extraction and intraocular IOL implantation,it is necessary to correct with the preoperative IOLMaster results.
