IVPA与PICC在宫颈癌化疗患者中的应用研究

摘　　要：目的探讨植入式静脉输液港(LVPA)与外周静脉穿刺中心静脉置管(PICC)在宫颈癌化疗患者中的应用效果,为临床置管应用提供参考。方法选择2015年1月到2016年12月在西安交通大学第一附属医院和陕西省咸阳市中心医院收治的725例宫颈癌化疗患者为研究对象,依照家庭经济情况及患者、患者家属的意愿,分为IVPA组(305例)和PICC组(420例)。比较两组的置管成功率、导管留置时间、导管维护操作时间、导管相关并发症的发生率、患者的置管舒适度及置管的费用等。结果IVPA与PICC相比,置管成功率显著增高(χ~2=4.508,P<0.05)、导管留置时间显著增加(t=52.745,P<0.05)、导管维护操作时间显著减少(t=13.110,P<0.05)、导管相关并发症的发生率显著降低(χ~2=26.36,P<0.05)、患者带管期间的置管舒适度显著增加(t=21.399,P<0.05)及置管费用显著减少(t=66.868,P<0.05)。结论对于宫颈癌化疗患者,推荐使用IVPA法,该方式可降低并发症发生率,减少护士工作量及工作强度,提高患者的置管舒适度,降低医疗成本。Objective To explore the application effects of implantable venous port access（ IVPA） and peripherally inserted central catheter（ PICC） on cervical cancer patients undergoing chemotherapy so as to provide some reference for clinical cathetering. Methods Totally 725 cases of cervical cancer in the First Affiliated Hospital of Medical College of Xi＇an Jiaotong University and Central Hospital of Xianyang City from January 2015 to December 2016 were selected as study subjects. According to the willingness of patients and their family members as well as incomes,the patients were divided into IVPA group（ 305 cases） and PICC group（ 420 cases） respectively. The following items were compared between two groups： success rate of inserted catheter,catheter indwelling time,catheter maintenance operation time,incidence of catheter related complications,patients＇ comfort with catheter and catheter expenses. Results Compared with PICC,IVPA has the advantages of increased success rate of inserted catheter（ χ~2 = 4. 508,P 0. 05）,prolonging catheter indwelling time（ t = 52. 745,P 0. 05）,reduced maintenance operation time（ t = 13. 110,P 0. 05）,decreased incidence of catheter related complications（ χ~2 = 26. 36,P 0. 05）,improved patients＇ comfort of catheter（ t = 21. 399,P 0. 05） and reduced expenses（ t =66. 868,P 0. 05）. Conclusion For cervical cancer patients with chemotherapy,IVPA is recommended. This method can reduce incidence of complications,reduce workload of nurses,improve patients＇ comfort of catheter,and reduce medical expenses.
