摘 要:目的探讨以问题为基础的学习法(PBL)联合腹腔镜技术在肝胆外科住院医师规范化培训临床带教中的应用价值。方法将2014级至2016级在肝胆外科轮转的外科学住院医师规范化培训学员共80名随机分为两组,观察组40名采用PBL教学法联合腹腔镜技术带教,对照组40名采用传统教学模式带教,采用理论和技能考核及问卷调查的方式对教学效果进行主客观评价。结果观察组学员的理论考试成绩[(85.5±3.00)分vs.(70.85±2.47)分]和技能考核成绩[(86.00±2.00)分vs.(70.30±1.73)分],均高于对照组(P<0.05),观察组学员在学习积极性,临床应变能力,团队意识,腹腔镜下感知能力,腹腔镜技能及带教满意度方面优于对照组(P<0.05)。结论 PBL教学法联合腹腔镜技术可提高学员学习兴趣,临床能力,团队意识和腹腔镜技能,有助于规培学员的临床带教。Objective To explore the application value of problem based learning(PBL) combined with laparoscopy in clinical training of standardized training for department of hepatobiliary surgery residents.Methods A total of 80 standardized surgical trainees from grade 2014 to grade 2016 in department of hepatobiliary surgery were randomly divided into two groups,observation group(40 cases) was taught PBL teaching method combined with laparoscopic teaching,control group(40 cases) was taught the traditional teaching mode,subjective and objective evaluation of teaching effectiveness was conducted by means of theoretical and technical assessment and questionnaire survey.Results The theoretical examination scores of the students in the observation group [(85.5±3.00) vs.(70.85±2.47)],skill examination result [(86.00±2.00) vs.(70.30±1.73)],were higher than the control group(P〈0.05),Observation group students in learning initiative,clinical strain capacity,team consciousness,perceived ability under laparoscopy,laparoscopic skills and satisfaction with teaching were better than those in the control group(P〈0.05).Conclusion PBL teaching combined with laparoscopic technology can improve the students' learning interest,clinical ability,team consciousness and laparoscopic skills,and help the clinical teaching of clinical trainees.
