小牛脾提取物联合奥沙利铂方案治疗晚期结肠癌的临床观察

摘　　要：目的探讨小牛脾提取物联合奥沙利铂方案治疗晚期结肠癌的疗效以及对T淋巴细胞亚群的影响。方法本研究收集晚期结肠癌患者95例,随机分为对照组47例和观察组48例。对照组:给予奥沙利铂化疗方案。观察组:在对照组基础上予小牛脾提取物注射液,1次/d,连用8周。比较二组生活质量评分、不良反应及临床疗效。检测二组血清T淋巴细胞亚群水平。结果治疗后观察组症状指标评分明显低于对照组,功能指标评分显著高于对照组(P<0.01)。观察组总有效率为89.58%,明显高于对照组70.21%(P<0.05)。观察组患者的不良反应发生率均少于对照组,且胃肠道反应、血小板减少、白细胞减少发生率明显少于对照组(P<0.05)。治疗后3个月,观察组患者的CD3+、CD4+水平明显高于治疗前和对照组同期水平,而CD8+显著减少,比较差异有统计学意义(P<0.01)。结论小牛脾提取物联合奥沙利铂方案治疗晚期结肠癌的疗效显著,上调患者的T淋巴细胞亚群水平可能与之有关。Objective To investigate efficacy of Calf spleen extractive combined with oxaliplatin scheme in treating carcinoma of colon and the influence on T lymphocyte cell subsets.Methods Ninty-five cases of carcinoma of colon were randomly divided into control group（47 cases）and observation group（48 cases）.Cases of control group were given with oxaliplatin scheme.Based on control group,observation group was treated with Calf spleen extractive,1 time/d and for 8 weeks.Scores of life quality,incidence rate of adverse drug reaction,and clinical effect were compared between the two groups.The influence on T lymphocyte cell subsets was detected in two groups.Results Scores of symptoms of observation group were lower than control group,and scores of function were higher than control group（P〈0.01）.Total effect rate of observation group was 89.58%,which was higher than control group as 70.21%（P 〈0.05）.Incidence rate of adverse drug reaction of observation group was lower than control group,and incidence rate of gastrointestinal reaction,thrombocytopenia,and leukocytopenia was obviously lower than control group（P〈0.05）.Levels of CD3＋,CD4＋ of observation group were higher,while CD8＋ was lower,than control group after 3 months’ treatment（P〈0.01）.Conclusion Efficacy of Calf spleen extractive combined with oxaliplatin scheme in treating carcinoma of colon is definite,and may be related to up-regulating T lymphocyte cell subsets
