血清总前列腺特异性抗原、血清游离前列腺特异性抗原与总特异性抗原的比值在前列腺癌中的诊断价值

摘　　要：目的探讨血清总前列腺特异性抗原(tPSA)、血清游离前列腺特异性抗原(fPSA)与总特异性抗原比值(f/tPSA)在前列腺癌中的诊断价值。方法回顾性分析2013年1月—2016年12月年间在唐山市工人医院行前列腺系统性穿刺活检或经尿道前列腺电切术的701例患者,按照病理结果将其分为前列腺癌组(Pca组)和前列腺增生组(BPH组),比较不同tPSA水平二组患者tPSA与f/tPSA比值的差异。结果当tPSA≤4ng/ml时,二组患者的tPSA、f/tPSA比值差异无统计学意义(P>0.05);当4ng/ml<tPSA≤10ng/ml时,二组患者tPSA差异无统计学意义(P>0.05),而f/tPSA比值在Pca组明显低于BPH组,差异有统计学意义(P<0.05);当tPSA>10ng/ml,二组患者的tPSA、f/tPSA比值的差异均具有统计学意义。结论 PSA是前列腺癌的肿瘤标志物,在测定tPSA的同时联合检测f/tPSA比值,有助于灰区内前列腺癌的诊断。Objective To explore the value of total prostate specific antigen（tPSA） and free to total PSA ratio（f/tPSA） in the diagnosis of prostate cancer（Pca）. Methods Totally 701 patients of prostatic diseases,who underwent prostate biopsy or transurethral resection of the prostate in author＇s hospital from Jan 2013 to Dec 2016, were selected into the study.The patients were divided into two groups,Pca group and benign prostatic hyperplasia （BPH） group, according to the pathologi- cal results.TPSA and f/tPSA in the patients were analyzed.Results When tPSA was below 4ng/ml, tPSA and f/tPSA in Pca group had no significant difference with those in BPH group （P 〉 0.05）.When it was between 4- 10ng/ml,tPSA in Pca group had no significant difference with that in BPH group（P 〉 0.05） ,but f/tPSA in Pea group was significantly lower than that in BPH group （P 〈 0.05）.When tPSA was higher than 10ng/ ml,tPSA and f/tPSA had significant differences between the two groups.Conclusion tPSA is a tumor marker of prostate cancer, and combining the detection of f/tPSA does favor to the diagnosis of prostate cancer within the gray zone.
