摘 要:目的:观察益气养阴、活血化瘀、清热解毒中药复方“益胃冲剂”对大鼠胃黏膜内源性PGE2及上皮细胞内粘蛋白的影响.方法:实验采用Indomethacin为胃黏膜损伤剂,米索前列醇(PGE1)为对照药物,实验动物为Ⅱ级♂SD大鼠;实验动物共分为两批,两批实验条件一致;每批完全随机化分为四组(n=32):控制组(S):0.9NaCl%+0.9NaCl%、损伤组(M):0.9NaCl%+吲哚美辛、益胃冲剂治疗组(YM):益胃冲剂+吲哚美辛、米索前列醇治疗组(PM):米索前列醇+吲哚美辛;所有大鼠24h禁食不禁水后,每组先后相隔30min两次经口灌胃给药;最后一次给药4h后,腹腔麻醉下手术摘除胃.第一批大鼠用于刮取胃黏膜,放射免疫检测胃黏膜内源性PGE2含量(ng/kg);第二批大鼠用于制作病理切片,在PAS—AB染色和PAS—AB—苏木精染色病理切片上,放大400倍,进行计算机图像定量分析胃黏膜表面上皮细胞内粘蛋白和黏膜内黏液指数(%).结果:胃黏膜内源性PGE2含量:益胃冲剂和米索前列醇均可保持PGE2含量明显高于Indomethacin损伤组(200.0±61.1pg/g&302.9±77.2pg/gvs35.6±16.5pg/g,P<0.01),但也明显低于控制组(vs636.9±104.9pg/g,P<0.01)的水平,胃窦部的结果与胃体部相似.在胃体部两治疗组组间比较有明显的差别(P<0.01),而在胃窦部却无差别;胃黏膜上皮细胞内粘蛋白指数:益胃冲剂可使粘蛋白保持在控制组和损伤组水平以上(41.1±3.3vs35.1±4.9&17.9±3.8,P<0.01),米索前列醇与益胃冲剂及控制组比较无差别(38.2±4.0vs41.1±3.3&35.1±4.9,P>0.05);胃黏膜内黏液指数:益胃冲剂可使黏液指数保持在与控制组及米索前列醇治疗组相当的水平(10.2±3.3vs11.7±3.0&7.8±2.1,P>0.05),上述三组与损伤组比较均有明显差异(vs3.8±1.3,P<0.01).结论:益胃冲剂对胃窦部较胃体部黏膜细胞有更好的促进PGE2合成作用,其效力相当于外源性给予米索前列醇的黏膜保护效力,而对胃体部黏膜细胞PGE2合成的促进作用�AIM: To observe the effect of Yiwei extract that supple-ments qi and nourishes yin on local defense mechanism ofgastric mucosa .METHODS: Thirty-two SD rats weighing 180-220g wererandomly classified into four groups: control groug (S), Yiweiextract treatment group (YM), indomethacin injury group(M) and misoprostol treatment group (PM). Indomethacin,a gastric mucosa injury agent, was injected into rat stom-ach orally. Saline solution, saline solution plus indomethacin,Yiwei extract plus indomethacin and misoprotol plus in-domethacin were given to group S, group M, group YM andgroup PM respectively . The stomachs were removed 4 hafter last administration. Endogenous prostaglandin E2( PGE2)concentration of gastric mucosa were measured byradioimmunoassay; mucin inside epithelial cells and mucusof gastric mucosa were assayed quantitatively by image-assaying.RESULTS:The content of the endogenous PGE2 in mucosaof gastric corpus was significantly higher ( P <0.01) in groupYM, compared with group M, and so did it in gastric antrum.The contents of mucin inside epithelial cells of gastric mucosawas significantly higher in group YM and PM than group M;the contents of the mucus inside gastric mucosa in group YMand group PM were higher than those in group M.CONCLUSION:Yiwei extract is able to effectively resistidomethacin-induced injury of gastric mucosa by means ofpromoting the production of endogenous PGE2 and the syn-thesis of mucin inside epithelial cells. The protective effecton gastric mucosa and mucin is similar to that of misoprostal.
