PCT及CRP对晚期肝病自发性细菌性腹膜炎患者的诊断及预后价值分析

摘　　要：目的探讨降钙素原(procalcitonin,PCT)及C反应蛋白(C reactive protein,CRP)对晚期肝病自发性细菌性腹膜炎(spontaneous bacterial peritonitis,SBP)患者的诊断及预后价值。方法收集重庆市巴南区人民医院2013年6月-2016年6月收治的晚期肝病SBP患者58例作为观察组,选取同期健康体检者50名作为对照组。采集两组血清标本测定PCT和CRP含量。且晚期肝病SBP患者采用常规治疗,观察治疗前后血清PCT和CRP含量变化,及预后情况。结果观察组血清PCT和CRP含量明显高于对照组,差异有统计学意义(P<0.05);晚期肝病SBP患者治疗后血清PCT和CRP含量低于治疗前,差异有统计学意义(P<0.05);生存患者血清PCT和CRP明显低于死亡患者,差异有统计学意义(P<0.05)。结论 PCT和CRP对晚期肝病SBP患者具有重要诊断价值,且可作为预后预测指标。Objective To investigate the values of procalcitonin （PCT） and C reactive protein （CRP） in the diagnosis and prognosis of spontaneous bacterial peritonitis （SBP） in patients with advanced liver disease. Methods Fiftyeight patients with advanced liver disease with SBP in Ba＇ nan People＇s Hospital of Chongqing from .lun. 2013 to Jun. 2016 were selected as the observation group, and at the same time, healthy subjects were selected as the normal group. Serum PCT and CRP levels were compared between two groups. Advanced liver disease with SBP were treated with con- ventional therapy, and the changes of serum PCT and CRP levels before and after treatment were observed. Results The serum levels of PCT and CRP were significantly higher than those in control group （P 〈 O. 05）. In observation group, the serum levels of PCT and CRP were lower after the treatment than before the treatment （ P 〈 0.05 ） ; the sermn levels of PCT and CRP in serum of patients with survival were significantly lower than those of patients who died （P 〈 0.05）. Conclusion PCT and CRP have important diagnostic values in patients with advanced liver disease with SBP, and can be used as prognostic indicators.
