DSOX与SOX两种方案治疗晚期复发性胃癌的疗效对比研究

摘　　要：目的研究DSOX和SOX两种方案对晚期复发性胃癌的治疗效果,为临床晚期复发性胃癌的治疗提供依据。方法对2013年2月至2015年10月该院入院治疗的130例晚期复发性胃癌患者进行研究,分为两组,对照组采用SOX治疗方案(奥沙利铂联合替吉奥)进行治疗,观察组采用DSOX治疗方案(多西他赛、奥沙利铂和替吉奥联合治疗)。比较两组治疗方案的远期疗效,两组患者的生活质量评分,两组治疗方案的临床控制率,以及两组患者发生Ⅰ~Ⅱ级不良反应的比例。结果观察组患者总生存期(OS)中位数为13.1个月,明显高于对照组(10.1个月),两组比较差异有统计学意义(P<0.05);观察组患者生活质量评分有效率可达87.80%,对照组仅为67.65%,两组比较差异有统计学意义(P<0.05);观察组患者临床控制率为85.1%,高于对照组(74.3%),两组比较差异有统计学意义(P<0.05);两组患者主要不良反应表现为白细胞减少、血小板减少、恶心呕吐,以及口腔黏膜炎等,其中观察组患者Ⅰ~Ⅱ级血小板减少发生率为2.7%,对照组患者Ⅰ~Ⅱ级血小板减少发生率为20.3%,观察组明显低于对照组(P<0.05)。结论 DSOX方案治疗晚期复发性胃癌临床效果显著,改善患者生活质量更明显,患者远期疗效更高,且不良反应发生比例更低,具有良好的治疗效果,值得推广。Objective To study the effect of DSOX and SOX on the treatment of recurrent gastric cancer,and provide the reference for advanced recurrent gastric cancer.Methods 130 patients in our hospital from Feb 2013 to Oct 2015 were studied,they were divided into two groups,the control was treated with SOX,for Oxaliplatin and Tegafur Gimeracil Oteracil Potassium Capsule combination,the observation group was treated with the DSOX,for Docetaxel,Oxaliplatin and Tegafur Gimeracil Oteracil Potassium Capsule combination treatment,the long curative effect,the quality of life score,clinical control rate and the adverse effect with gradeⅠ―Ⅱ were compared in the two groups after treatment.Results The median of OS was 13.1 months in the observation group,which significantly higher than control group of 10.1months（P0.05）;the score of quality life was 87.80%,the control was only 67.65%（P0.05）;the clinical effect was 85.10%in the observation group while the control was 74.30%（P0.05）;the major adverse reactions in two groups was leukopenia,thrombocytopenia,nausea and vomiting,and oral mucosa inflammation etc.,the incidence of gradeⅠ―Ⅱthrombocytopenia was 2.70%in the observation group,the control was 20.30%,which significantly lower than control（P0.05）.Conclusion The effect of DSOX on the advanced recurrent gastric cancer clinical effect was remarkable,the quality of life was improved obviously,the long curative effect was higher,and the adverse reactions were lower,it is worth promoting.
