LEEP术联合重组人干扰素α-2b凝胶治疗宫颈上皮内瘤变疗效分析

摘　　要：目的观察宫颈环形电极切除术(LEEP)联合重组人干扰素α-2b凝胶治疗宫颈上皮内瘤变(CIN)的临床效果。方法 76例行LEEP术的CIN患者,随机分为对照组和研究组,各38例。两组术前均行人乳头瘤病毒(HPV)检测,均采用LEEP刀治疗,对照组术后给予抗生素治疗,研究组在对照组的基础上采用重组人干扰素α-2b凝胶治疗。比较两组临床疗效、创面愈合时间、并发症发生情况及HPV转阴率。结果研究组患者总有效率为94.7%,明显高于对照组的78.9%,差异具有统计学意义(P<0.05);研究组患者创面愈合时间明显短于对照组,差异具有统计学意义(P<0.05);研究组患者术后并发症发生率为10.5%,明显低于对照组的28.9%,差异具有统计学意义(P<0.05);术后12周,研究组患者HPV转阴率为92.1%,明显高于对照组的73.7%,差异均具有统计学意义(P<0.05)。结论 LEEP术联合重组人干扰素α-2b凝胶治疗CIN效果理想,能够有效缩短创面愈合时间,减少术后并发症的发生,提高HPV的转阴率,值得推广应用。Objective To observe the clinical effect of loop electrosurgical excision procedure（LEEP） combined with recombinant human interferon α-2b gel in the treatment of cervical intraepithelial neoplasia（CIN）. Methods A total of 76 cervical intraepithelial neoplasia patients with LEEP surgery were randomly divided into control group and research group, with 38 cases in each group. Both groups were detected with human papillomavirus（HPV） detection before operation, and they were treated with LEEP knife. The control group was treated with antibiotics after operation, and the research group was treated with recombinant human interferon α-2b gel on the basis of the control group. Comparison were made on clinical efficacy, wound healing time, complications and HPV negative-conversion rate in two groups. Results The research group had obviously higher total effective rate as 94.7% than 78.9% in the control group, and the difference was statistically significant（P〈0.05）. The research group had obviously shorter wound healing time than the control group, and the difference was statistically significant（P〈0.05）. The research group had obviously lower incidence of postoperative complications as 10.5% than 28.9% in the control group, and the difference was statistically significant（P〈0.05）. In postoperative 12 weeks, the research group had obviously higher HPV negative-conversion rate as 92.1% than 73.7% in the control group, and their difference was statistically significant（P〈0.05）. Conclusion Combination of LEEP surgery and recombinant human interferon α-2b gel provides ideal effect in treating CIN, and it can effectively shorten the wound healing time, reduce the incidence of postoperative complications, improve HPV negativeconversion rate. So it is worthy of promotion and application.
