摘 要:目的探讨帕金森病(PD)患者的血清尿酸水平与运动障碍及左旋多巴治疗前后的关系。方法 126例PD患者(PD组)及120例健康对照者(健康对照组)分别测定血清尿酸水平。PD患者服用左旋多巴6个月后分为有效组(96例)和无效组(30例),比较有效组与无效组用药前后血清尿酸水平。采用Spearman分析法分析患者血清尿酸水平与疾病严重程度[采用Hoehn-Yahr(H-Y)分级量表评估]的关系。结果 PD组血清尿酸水平为(208.3±86.5)μmol/L,明显低于健康对照组的(298.7±66.4)μmol/L,差异具有统计学意义(P<0.01)。女性PD患者血清尿酸水平低于男性PD患者,但差异无统计学意义(P>0.05)。Spearman等级相关分析结果显示,患者血清尿酸水平与患者的Hoehn-Yahr分级呈负相关(r=-0.442,P<0.05)。服用左旋多巴制剂6个月后有效组患者用药后的血清尿酸水平明显高于用药前,差异具有统计学意义(P<0.05)。服用左旋多巴制剂6个月后无效组患者用药后的血清尿酸水平略低于用药前,但差异无统计学意义(P>0.05)。结论低血清尿酸水平可能是PD的危险因素之一,血清尿酸水平或许可以评定PD患者病情严重程度,高血清尿酸水平可能会减轻PD的氧化应激损伤。Objective To investigate the correlation of serum uric acid level with movement disorder and levodopa before and after treatment in Parkinson' s disease (PD) patients. Methods There were 126 PD patients (PD group) and 120 healthy controls (health control group), and the serum uric acid levels were measured. Spearman analysis was used to analyze the correlation between serum uric acid levels and disease severity [ evaluated by Hoehn- Yahr (H-Y) grading scale ] . Results PD group had obviously lower serum uric acid levels as (208.3 ± 86.5) μmol/L than (298.7±66.4) μmol/L in healthy control group, and the difference was statistically significant (P〈0.01). Female PD patients had lower serum uric acid levels than male PD patients, but the difference was not statistically significant (P〉0.05). The results of Spearman rank correlation analysis showed that serum uric acid levels were negatively correlated with Hoehn-Yahr grading in patients (r=-0.442, P〈0.05). After administration of levodopa for 6 months, the effective group had obviously higher serum uric acid levels after administration than before administration, and the difference was statistically significant (P〈0.05). After administration of levodopa for 6 months, there were 30 cases in ineffective group had slightly lower higher serum uric acid levels after administration than before administration, but the difference was not statistically significant (P〉0.05). Conclusion Low serum uric acid levels may be one of the risk factors for PD, and serum uric acid levels may be able to assess the severity Of PD patients. High serum uric acid levels may reduce PD oxidative stress injury.
