探究异常分娩中臀位助产术的临床应用效果

摘　　要：目的探究异常分娩中臀位助产的临床应用效果。方法选取2015年2月—2016年2月该院收治的异常分娩产妇130例,其中采取臀位助产术40例,剖宫产术60例,臀位牵引术30例,比较各组胎儿并发症发生情况和死亡率,分析应用效果。结果胎儿体质量低于3 500 g大多选择臀位助产术,体质量达3 500 g及以上大多选择剖宫产术,臀位助产术分娩胎儿并发症发生率(10.00%)、死亡率(2.50%)较剖宫产分娩(33.33%、3.33%)与臀位牵引术分娩(40.00%、6.67%)胎儿低,其中臀位牵引术分娩胎儿并发症发生率、死亡率最高,并发症发生率比较,差异有统计学意义(P<0.05)。结论在异常分娩中需综合评估产妇与胎儿实际情况后选择合适的助产方式,臀位助产适用于体质量低于3 500 g的异常分娩中,可减少胎儿并发症的发生。Objective To study the clinical application effect of partial breech extraction in the abnormal labor. Methods 130 cases of delivery women with abnormal labor admitted and treated in our hospital from February 2015 to February 2016 were selected and 40 cases, 60 cases, 30 cases were respectively treated with partial breech extraction, cesarean section and breech extraction, and the oc- currence of complications and morality were compared between the three groups, and the application effect was analyzed. Results Most of fetuses whose body mass was less than 3 500 g selected the partial breech extraction, and the fetuses whose body mass was more than 3 500 g mostly selected the cesarean section and the incidence rate of complications and morality of fetuses treated with partial breech extraction were lower than those of fetuses treated with cesarean section and breech extraction （10.00%, 2.50% vs 33.33%, 3.33% vs 40.00%, 6.67%）, and the differences were statistically significant （P〈O.05）. Conclusion We need to select the proper mid- wifery mode after the comprehensive evaluation of practical conditions of delivery women and fetuses, and the partial breech extrac- tion is suitable for the abnormal delivery of fetuses whose body mass is less than 3 500 g thus reducing the occurrence of fetal compli- cations.
