摘 要:目的探讨吉西他滨在预防家兔后发性白内障中的安全性和有效性。方法 36只3月龄新西兰白兔,雌雄不限,随机分为空白对照组、低剂量组(5 mg/ml)和高剂量组(10 mg/ml)三组,每组12只(12只眼);实验动物均行白内障囊外摘除+人工晶体植入术;术后1,3,5,7 d监测角膜水肿,前房闪辉变化;术后30 d对晶状体后囊膜混浊(PCO)及角膜内皮细胞数量进行分析。结果术后第1,3天,三组均出现不同程度角膜水肿、前房闪辉;术后第5天,三组角膜水肿吸收;第7天前房闪辉大部吸收,角膜水肿、前房闪辉组间比较差异无统计学意义(P>0.05)。角膜内皮细胞计数显示各组术后30 d角膜内皮细胞数量较术前下降,角膜内皮丢失量三组间比较,差异无统计学意义(P>0.05)。术后第2周三组均出现后囊膜混浊,术后30d PCO分级比较,空白对照组多分布于2级和3级,低剂量组和高剂量组后囊膜混浊分布于1级和2级,空白对照组PCO较低剂量组和高剂量组明显,差异有统计学意义(P<0.01)。结论吉西他滨可以有效预防后囊膜混浊的发生,眼内应用安全,未发现角膜内皮细胞数量改变。Objective To investigate the safety and efficacy of gemcitabine for preventing posterior capsule opacification( PCO) in rabbit eyes. Methods Thirty-six 3-month-old New Zealand rabbits were randomly and equally divided into three groups: blank control group,low dose gemcitabine( 5 mg/ml) group and high dose gemcitabine( 10 mg/ml) group. All rabbits underwent extracapsular cataract surgery( ECCE) and intraocular lens( IOL) implantation. At 1,3,5 and 7 d after operation,the changes of corneal edema and anterior chamber flare were observed. At 30 d after operation,the posterior capsule opacification and the change of cornea and anterior chamber's exudation were observed and compared among the three groups. Results At 1,3,5 and 7 d after operation,corneal edema and anterior chamber flare occurred in various degree in three groups. The corneal edema disappeared on postoperative day 5. The anterior chamber flare was absorbed in most cases on postoperative day 7. The postoperative corneal edema and anterior chamber flare showed no statistical significance among the three groups( P〈0. 05).The corneal endothelium cell density in the three groups on postoperative day 30 was lower than that before operation,and the loss of corneal endothelium cell density was of no significant difference among the three groups( P〈0. 05).The posterior capsule opacification appeared at 2 week after operation. On postoperative day 30,the posterior capsule opacification was grade 1 and grade 2 in low dose group and high dose group,and grade 2 and grade 3 in blank control group.The grade of posterior capsule opacification was significantly lower in low dose group and high dose group than in blank control group( P〈0. 01). Conclusion The results suggest that gemcitabine could effectively and safely prevent the formation and development of posterior capsule opacification in rabbits.
