替吉奥与小牛脾注射液联合治疗进展期胃癌的疗效及对MMP-9和VEGF表达的影响

摘　　要：目的探讨替吉奥与小牛脾注射液联合治疗进展期胃癌的疗效及对MMP-9和VEGF表达的影响。方法选取80例进展期胃癌患者,按随机分组法将所有患者分为观察组和对照组,每组40例。观察组采用替吉奥、奥沙利铂与小牛脾注射液联合治疗,对照组采用替吉奥与奥沙利铂联合治疗。比较2组患者临床疗效、生存率、不良反应及化疗前后MMP-9及VEGF灰度值。结果观察组的总有效率(62.5%)显著高于对照组(32.5%)(P<0.05)。观察组的1、2年生存率显著高于对照组(P<0.05)。在Ⅰ~Ⅱ级不良反应中,观察组患者恶心呕吐、腹泻、白细胞下降、红细胞下降均显著低于对照组(P<0.05),但在血小板下降和肝功能异常方面2组差异无统计学意义(P>0.05)。化疗后2组患者MMP-9及VEGF灰度值均显著降低(P<0.05),观察组灰度值显著低于对照组(P<0.05)。结论替吉奥与小牛脾注射液联合治疗进展期胃癌的疗效显著,可有效提高生存率,降低不良反应,值得推广。Objective To investigate the efficacy of S-1 combined with calf spleen injection in the treatment of advanced gastric cancer and its effect on the expression of MMP-9 and VEGF. Methods 80 patients with advanced gastric carcinoma were selected. All patients were randomly divided into the observation group and the control group,40 cases in each group. The observa- tion group received S-1 and oxaliplatin injection combined treatment with calf spleen, the control group received S-1 combined with oxaliplatin for treatment. The clinical efficacy, survival rate, adverse reactions and MMP-9 and VEGF values before and after chemotherapy were compared between the 2 groups. Results The total effective rate of the observation group（62.5% ） was sig- nificantly higher than that of the control group （32.5 % ） （ P 〈 0.05 ）. 1 -, 2-year survival rates of the observation group were sig- nificantly higher than those of the control group（P 〈0.05）. In the I - I1 adverse reactions nausea and vomiting,diarrhea,de- crease of white blood cells, red blood cells in the observation group were significantly lower than the control group （ P 〈 0.05 ） , decreased platelet count and liver function abnormalities had no statistically significant （ P 〉 0.05 ）. After chemotherapy, the MMP- 9 and VEGF gray values of the 2 groups were significantly lower （ P 〈 0. 05 ） , the observation group was significantly lower than that of the control group （ P 〈 0.05 ）. Conclusion S-1 and calf spleen injection in the treatment of advanced gastric cancer have significant effect, can effectively improve the survival rate, reduce the adverse reaction, is worthy of promotion.
