原发性肝癌合并门静脉瘘TACE药代动力学影响因素的研究

摘　　要：目的研究与分析原发性肝癌合并门静脉瘘TACE药代动力学影响因素。方法选取行TACE治疗的30例原发性肝癌合并门静脉瘘患者为研究组,同时期的30例无门静脉瘘患者为对照组,然后统计与比较两组的临床总有效率、不同时间的洛铂外周静脉血药浓度、药代动力学参数,并比较研究组中不同严重程度与瘘口处理方式患者的上述检测结果。结果研究组的临床总有效率低于对照组,不同时间的洛铂外周静脉血药浓度高于对照组,药代动力学参数均低于对照组,且研究组中不同严重程度与瘘口处理方式患者的上述检测结果也存在显著性差异,P均<0.05。结论门静脉瘘对原发性肝癌患者的TACE治疗效果有一定的影响,而门静脉瘘的严重程度与瘘口处理方式对患者的药代动力学影响也较大,应给予充分重视。Objective To study and analyze the influence factors of pharmacokinetics of patients with primary hepatocel- lular carcinoma with portal vein fistula treated with TACE. Methods 30 patients with primary hepatoeellular carcinoma with por- tal vein fistula treated with TACE were selected as the study group ,30 patients without portal vein fistula were selected as the con- trol group, the total effective rates, peripheral venous blood concentration of lobaplatin at different time, pharmacokinetic parame- ters of the 2 groups were analyzed and compared, and those detection results of the study group with different severity degree and fistula treatment methods were compared too. Results The total effective rate of the study group was lower than that of the control group, the peripheral venous blood concentration of lobaplatin at different time were all higher higher than those of the control group, the pharmacokinetic parameters were all lower than those of the control group, and those detection results of the study group with different severity degree and fistula treatment methods had significant difference, all P 〈 0.05, there were significant differ- ence. Conclusion The portal vein fistula has certain influence for primary hepatocellular carcinoma treated with TACE, and the influence of severity degree and fistula treatment methods for the pharmacokinetics is greater, it should be paid fully attention.
