等离子体电切术与电汽化术治疗老年膀胱癌的疗效对比

摘　　要：目的对等离子体电切术与电汽化术治疗老年膀胱癌的疗效进行分析与对比,以期对临床老年膀胱癌手术方式的选择提供指导。方法对132例老年膀胱癌患者的临床资料进行回顾性分析,根据手术方式的不同将其分为观察组70例及对照组62例。观察组患者行等离子体电切术,对照组患者行电汽化术。对2组患者手术情况、并发症发生率及患者生活质量进行比较。结果 2组患者手术时间及术中出血量无统计学差异(P>0.05)。观察组患者术后肉眼血尿时间、留置导尿管时间以及住院时间均显著短于对照组,差异有统计学意义(P<0.05)。观察组患者术后并发症发生率为2.86%,低于对照组的14.52%,差异有统计学意义(P<0.05)。2组1年复发率分别为1.43%和3.23%,差异无统计学意义(P>0.05)。2组患者手术后生活质量各维度得分均优于术前,且随术后时间的延长生活质量得分越高(P<0.05);术后各时间点观察组生活质量得分均高于对照组,差异有统计学意义(P<0.05)。结论等离子体电切术治疗老年膀胱癌具有良好的临床疗效,并且安全性较高,患者预后较好,值得临床推广应用。Objective To explore the clinical effect of transurethral plasma kinetic resection and vaporization in the treatment of elderly patients with bladder tumor, and provide basis for the treatment of elderly patients with bladder tumor. Methods 132 elderly patients with bladder tumor were retrospectively analyzed and divided into the observation group （70 cases）and the control group （62 cases）according to different methods. Patients in the observation group underwent transurethral plasma ki- netic resection, and patients in the control group underwent vaporization. Then the operation situations, complications and life qual- ity of the 2 groups were compared. Results The operation time and blood loss of the 2 groups had no significant difference （P 〉 0.05）. The gross hematuria time,indwelling catheter time and hospital stays of the observation group were obviously shorter than the control group with statistical significance （P 〈 0.05 ）. The complication rate of the observation group was 2.86%, which was obviously lower than 14.52% of the control group, there had statistical difference （P 〈 0.05 ）. The recurrence rates of the 2 groups were 1.43 % and 3.23% , there had no statistical difference （P 〉 0.05 ）. The life quality after operation of the 2 groups were all getting better than before with the time extended （ P 〈 0. 05 ）. The life quality of the observation group after surgery were all better than the control group,, there had statistical difference （ P 〈 0.05 ）. Conclusion Transurethral plasma kinetic resection has good efficacy and safety in the treatment of elderly patients with bladder tumor, it is good at prognostic improvement and is worthy of clinical applications.
