摘　　要：目的评价我国急性白血病(AL)住院患者发生医院感染的主要危险因素。方法系统检索PubMed、中国期刊全文数据库(CNKI)、中国生物医学文献数据库、万方数据库以及维普信息资源系统中建库至2017年4月发表的与AL患者发生医院感染危险因素有关的研究文献;2名研究员独立按照纳入与排除标准筛选文献、提取资料及质量评价后,采用RevMan5.3软件进行Meta分析。结果最终纳入文献14篇,累计AL患者5350例;Meta分析显示,住院天数、中性粒细胞计数、年龄、粒缺持续时间、糖皮质激素应用史、治疗阶段、住院季节与AL患者发生医院感染相关。结论住院天数、中性粒细胞计数、年龄、粒缺持续时间、糖皮质激素应用史、治疗阶段、住院季节是我国AL患者发生医院感染的主要危险因素。OBJECTIVE To evaluate the major risk factors for nosocomial infections in hospitalized patients with acute leukemia（AL）in China.METHODS The PubMed,CNKI,CBM,Wanfang database and VIP information resource system were retrieved for the research literatures regarding the risk factors for nosocomial infections in AL patients that were published from the establishment of database to Apr,2017.The literatures were screened independently by 2 researchers on the basis of inclusion and exclusion criteria,the data were extracted,the quality was evaluated,and the meta-analysis was performed with the use of RevMan5.3 software.RESULTS Totally 14 literatures were included,involving 5350 AL patients in total.The meta-analysis indicated that the incidence of nosocomial infections in the AL patients was associated with length of hospital stay,neutrophil counts,age,duration of neutropenia,history of use of glucocorticoids,treatment stage and season of hospitalization.CONCLUSION The major risk factors for nosocomial infections in the AL patients in China include the length of hospital stay,neutrophil counts,age,duration of neutropenia,history of use of glucocorticoids,treatment stage and season of hospitalization.
