hMAM在乳腺癌前哨淋巴结中的表达及其临床意义分析

摘　　要：探讨人乳腺球蛋白(human mammary globulin,hMAM)在乳腺癌前哨淋巴结(sentinel lymph node,SLN)中的表达及其临床意义。RT-PCR检测20例乳腺癌组织及正常淋巴结组织中hMAM的表达;SLN定位后通过RT-PCR与常规病理检测SLN转移情况,并进行比较;分析SLN转移与临床病理的关系。乳腺癌组织中有18例过表达,表达率为90.0%,而正常淋巴结中hMAM不表达,两者差异有统计学意义(P<0.01)。常规病理检查转移率为50.98%,RT-PCR法检测的阳性率提高到70.59%,RT-PCR法比常规病理检测的检出率高19.61%,两者之间差异显著(χ2=38.28,P<0.01)。SLN无转移组、微转移组、转移组与肿瘤位置、ER表达及病理类型之间差异均不显著(P>0.05)。hMAM可作为辅助判断SLN是否转移的标志。To investigate the expression and clinical significance of human mammary globulin（hMAM）in sentinel lymph nodes（SLN）of breast cancer,the mRNA expression of hMAM was detected in 20 cases of breast cancer tissues and normal lymph node tissues by RT-PCR.SLN transfer after SLN localization were detected by RT-PCR and routine pathology,and the comparison was done to analyze the relationship between sentinel lymph node metastasis and clinical pathology.In breast cancer,hMAN were overexpressed in 18 cases,the expression rate was 90.0%,while the expression of hMAM in normal lymph nodes was not significant（P 〈 0.01）.The routine pathological examination showed a metastasis rate of 50.98%,while the positive rate was 70.59% by RT-PCR,which was 19.6% higher than that of routine pathological examination（χ2=38.28,P 〈 0.01）.There were no significant difference in no metastasis group,micro metastasis group and metastasis group of SLN and tumor location,ER expression and pathological type（P 〉 0.05）.These findings suggest hMAM can be used as a potential marker for judging the metastasis of SLN.
