军队离退休干部阿尔茨海默病患者临床转归的影响因素分析

摘　　要：目的分析影响阿尔茨海默病(AD)患者临床转归的影响因素。方法回顾2012年1月~12月在福州总医院住院的男性离退休干部轻度AD患者336例,随访5年,以死亡为终点事件选取AD患者116例为研究组,选择同期住院死亡的非AD男性患者210例作为对照组。回顾两组患者的一般资料和临床指标,单因素及多因素分析军队离退休干部AD患者临床转归的影响因素。结果两组一般资料比较,死亡年龄、高认知活动、运动、照顾质量、营养状态、抑郁症差异有统计学意义(P<0.05);临床资料比较,C-反应蛋白(CRP)、三碘甲状腺原氨酸(T3)、甲状腺素(T4)、游离三碘甲状腺原氨酸(FT3)、空腹血糖(FBG)、高密度脂蛋白(HDL)差异有统计学意义(P<0.05);Logstic回归分析显示,T3(OR=4.08,95%CI=1.44~14.78)、FT3(OR=25.0,95%CI=13.55~44.34)、FBG(OR=1.381,95%CI=1.005~1.898)是军队离退休干部AD患者临床转归的相关影响因素。结论 T3、FT3、FBG是军队离退休干部AD患者临床转归的相关影响因素,对阿尔茨海默病的临床治疗有潜在意义。Objective To investigate the clinical outcome factors influencing Alzheimer＇s disease（AD） patients. Methods336 cases of male veterans AD patients were involved in our hospital from January 2012 to December 2012 and fllowed up for 5years. 116 cases were selected as the study group and another 210 death participants in the same time were enrolled as the control group. All demography data and clinical data of the patients in both groups were retrospectively analyzed. Results Age at death,high cognitive activity,quality of care,stautes of nutrition,exercise and depression in both groups were all statistically significant（P〈0.05 or P〈0.01）. Levels of c-reactive protein（CRP）,liothyronine sodium（T3）,thyroxine（T4）,free triiodothyronine（FT3）,fast blood glucose（FBG）,high density lipoprotein（HDL） in the two groups were all statistically significant（P〈0.05 or P〈0.01）.The logistic regression analysis found that T3（OR=4.08,95%CI=1.44 - 14.78）、FT3（OR=25.0,95%CI=13.55 - 44.34）、FBG（OR=1.381,95%CI=1.005 - 1.898）were the relevant influence factors of male veterans AD patients. Conclusion T3、FT3 and FBG are the relevant influence factors of the male veterans Alzheimer＇s disease patients and may provide potential benefits in clinical treatment of Alzheimer＇s disease.
