黄芪对腹腔镜下胃大部切除术后胃癌患者T细胞亚群的影响

摘　　要：目的:探讨黄芪对腹腔镜下胃大部切除术后胃癌患者T细胞亚群的影响。方法:自2015年1月—2016年1月,前瞻性收集医院收治的腹腔镜下胃大部切除术后胃癌患者100例,将患者随机分为研究组和对照组,每组各50例。研究组术后给予黄芪注射液治疗联合常规治疗,对照组仅给予常规治疗。主要观察指标包括两组患者CD_4^+T细胞、CD_8^+T细胞、CD_4^+/CD_8^+T细胞、IL-2、IL-6和TGF-β1。结果:两组患者术前CD_4^+T细胞、CD_8^+T细胞、CD_4^+/CD_8^+T细胞、IL-2、IL-6和TGF-β1等差异无统计学意义(P>0.05)。与对照组比较,研究组患者术后14 d时CD_4^+T细胞水平显著升高[(34.39±4.82)%vs.(31.59±4.42)%,P=0.003];CD_4^+/CD_8^+T细胞比例显著升高[(1.46±0.24)%vs.(1.31±0.21)%,P=0.000];IL-2水平显著升高[(4.86±0.51)μg/L vs.(4.37±0.57)μg/L,P=0.001];IL-6水平显著升高[(25.83±4.82)ng/L vs.(21.73±5.39)ng/L,P=0.000];TGF-β1水平显著降低[(474.24±84.39)ng/L vs.(530.49±87.49)ng/L,P=0.000]。结论:黄芪联合胸腺五肽有助于改善胃癌患者腹腔镜下胃大部切除术后免疫抑制状态。Objective：To investigate the effect of Astragalus on T cell subsets in patients with gastric carcinoma after laparoscopic gastrectomy. Methods：From January 2015 to January 2016, 100 patients with gastric cancer after laparoscopic subtotal gastrectomy in our hospital were studied and randomly divided into a study group and a control group, with 50 cases in each group. The study group was treated with Astragalus Mongholicus Injection combined with routine treatmentand the control group was given routine treatment only. Main outcome measures included CD4^＋ T cells, CDs T cells, CD4^＋/CDs T cells, IL- 2, IL -6 and TGF - beta 1 in the two groups. Results：There were no significant differences in preoperative CD4^＋ T cells, CDs T cells, CD4^＋/CDs T cells, IL - 2, IL - 6 and TGF - beta 1 between the two groups （ P 〉 0.05 ）. When compared with the control group, the level of CD4^＋ T cells at day 14 after the operation were significantly higher in patients of study group[ （34.39 ± 4.82） % vs. （31.59 ±4.42） %, P = 0. 003 ） ]. The proportion of CD4^＋ / CDs T cells increased significantly [ （ 1.46 ±0.24） vs. （ 1.31 ±0.21 ）, P = 0. 000） ]. IL - 2 level increased significantly[ （4.86 ±0.51 ） ~g/L vs. （g. 37 ±0.57） μg/L, （P = 0. 001 ） ]. The level of IL - 6 increased significantly [ （25.83 ±4.82） ng/L vs. （21.73 ±5.39） ng/L, （P =0. 000） ]. TGF - beta 1 was significantly lower[ （474.24 ±84.39） ng/ Lvs. （530.49 ±87.49） ng/L, P = 0. 000） ]. Conclusion ： Astragalus combined with thymus peptide five is helpful to improve the immune suppression status after laparoscopic subtotal gastrectomy in patients with gastric cancer.
