经阴道和经腹超声在诊断良性子宫肌瘤中的临床价值分析

摘　　要：目的探究经阴道超声和经腹超声在诊断良性子宫肌瘤上的临床应用价值。方法将我院妇科收治的413例良性子宫肌瘤患者(632个肌瘤)作为研究对象,对其经阴道超声和经腹部超声结果等影像学资料进行回顾性分析。依据病理学结果对经阴道超声、经腹部超声以及联合诊断的诊断结果进行分析比较。结果除浆膜下肌瘤以外,经阴道超声在诊断不同类型子宫肌瘤上的诊断准确率均高于经腹部超声,差异比较有统计学意义(P<0.05)。经阴道超声和经腹部超声在诊断子宫肌瘤上的诊断准确率分别为93.51%和73.10%。联合诊断的诊断准确率为99.53%,准确率高于单一诊断方式(P<0.05)。结论在超声诊断良性子宫肌瘤上,经阴道超声的诊断准确率高于经腹部超声。两种超声诊断的联合应用,进一步提高其临床诊断准确率,减少子宫肌瘤漏诊和误诊情况的发生。Objective To research into the clinical value of transvaginal and transabdominal uhrasonography in diagnosis of benign uterine leiomyoma. Methods Totally 413 patients with benign uterine leiomyoma （632 fibroids） in the department of obstetrics and gynecology in our hospital were selected as the research objects. We Analyzed the results of transvaginal ultra- sound and transabdominal ultrasound retrospectively. According to the pathological results, we analyzed and compared the results of transvaginal ultrasound, transabdominal ultrasound and combined diagnosis. Results In the diagnosis of different types of u- terine fibroids, the diagnostic accuracy of transvaginal ultrasound was significantly higher than that of transabdominal ultrasound besides the subserous myoma, and the difference was statistically significant （P〈0.05）. The diagnostic accuracy of transvaginal ultrasound and transabdominal ultrasound in the diagnosis of uterine fibroids was 93.51% and 73.10%, respectively, and the di- agnostic accuracy raie of combined diagnosis was 99.53%, which was significantly higher than that of single diagnosis （P〈 0. 05）. Conclusion In the ultrasonic diagnosis of benign uterine teiomyoma, the diagnostic accuracy of transvaginal ultrasound was significantly higher than that of transabdominal ultrasound. The combined application of the two kinds of ultrasonic diagnosis further improves the accuracy of clinical diagnosis, and avoids the misdiagnosis and misdiagnosis of uterine fibroids to a great ex- tent.
