摘　　要：目的利用高分辨率磁共振评价不同剂量瑞舒伐他汀钙对脑梗死患者颈动脉硬化斑块的影响。方法将经影像学检查证实有颈动脉硬化斑块的脑梗死的住院患者43例随机分为高剂量组和低剂量组，分别给予瑞舒伐他汀钙20mg／d和10mg／d口服，比较两组患者治疗前和治疗后1年血脂、血清超敏C-反应蛋白（hs-CRP）水平和硬化斑块体积、成分的变化。结果治疗后1年，两组患者血脂总胆固醇、低密度脂蛋白胆目醇（LDL-C）、hs-CRP水平均降低，高密度脂蛋白胆固醇（HDL-C）水平升高，与治疗前比较，差异有统计学意义（P〈0．05）；经高分辨核磁共振检查，颈动脉硬化斑块的体积及脂质核心体积百分比与治疗相比较均有降低。治疗后，高剂量组患者总胆固醇、LDL—C水平、脂质核心体积与低剂量组比较，差异有统计学意义（P〈0．05）。结论瑞舒伐他汀钙能改善患者的血脂、hs—CRP水平，且具有逆转颈动脉硬化斑块的作用。Objective To evaluate the effect of rosuvastatin calcium on carotid plaque in cerebral infarction patients by high resolution magnetic resonance. Methods Forty-three patients who had confirmed carotid plaques by imaging examination were randomly divided into two groups, the high-dose group was treated with rosuvastatin calcium 20 mg/d, while the low-dose group was treated with rosuvastatin calcium 10 mg/d. The changes of blood lipid, serum high sensitive C-reactive protein（hs-cre） level, the volume and composition of atherosclerotic plaques in the two groups were compared before and after treatment for one year. Results After one year＇ s treatment with different dose of rosuvastatin calcium, the blood lipid levels in patients such as total cholesterol （TC）, low density lipoprotein cholesterol （LDL-C） and hs-CRP decreased, and hihg density lipoprotein cholesterol （HDL-C）Levels increased, compared with before treatment, the differences were significant （ P 〈 0. 05 ）. The volume of carotid plaques and lipid cores increased by high resolution nuclear magnetic resonance （MRI 3.0T）. After treatment, the TC, LDL-C, and the volume of lipid core in the high dose group were significantly different from those in the low dose group （P 〈 0. 05 ）. Conclusions Rosuvastatin calcium can decrease the blood lipid, C- reactive protein level and have anti atherosclerosis effect.
