摘　　要：目的通过临床药师实践过程,探索临床药师的工作方式及思维方法。方法参与1例心衰伴高钾血症的治疗,充分利用专业背景,在高钾血症查因、药物不良反应的监测等环节,作为临床治疗团队一员,为患者提供全面的药学服务。结果临床药师参与的临床治疗团队能更好地为患者提供服务,该心衰患者在优化治疗方案后能避免血钾升高。结论临床药师参与临床治疗,有利于发挥药师在治疗团队中的作用,促进临床合理用药,保障患者用药安全。Objective To explore the working and thinking mode of clinical pharmacists in clinical practice. Methods Clinical pharmacists joined the treatment team and provided pharmaceutical care for a heart failure patient with hyperkalemia, such as checking the hyperkalemia, and monitoring the adverse drug reaction （ADR）. Results The treatment team provided better care with the help of clinical pharmacists. The heart failure patient avoided potassium rise after optimization of the treatment. Conclusion Involvement of clinical pharmacists in therapeutic practice improves the security and efficacy of pharmacotherapy.
