摘  要:目的:探讨急性髓系白血病(AML)患者接受诱导化疗后第14天淋巴细胞绝对数(ALC)对预后的影响。方法:回顾性分析54例AML患者,比较诱导化疗后完全缓解组与未缓解组化疗后第14天ALC;根据诱导化疗后第14天ALC中位数将患者分为两组,比较两组患者的总体生存率(OS)、无病生存率(DFS)、复发率及非复发死亡率(NRM)。结果:完全缓解组诱导化疗后第14天ALC高于未达到完全缓解组;高ALC组DFS均显著高于低ALC组,复发率显著低于低ALC组。结论:AML患者诱导化疗后第14天ALC与预后相关,淋巴细胞水平检测在评价AML疗效及判断预后方面具有一定的临床价值。Objective:To investigate the impact on prognosis of absolute lymphocyte counts at day 14 after receiv- ing standard induction and priming induction regimen in patients with acute myeloid leukemia(AML). Methods :The data of 54 patients with AML receiving standard induction regimen were included. The absolute lymphocyte counts (ALC) at day 14 after induction chemotherapy were analyzed between complete remission group and no remission group. According to the median number of ALC at day 14 after induction chemotherapy, patients were grouped into high ALC group and low ALC group. Prognosis outcomes, including overall survival rate (OS) , disease free survival rate( DFS), relapse rate and non- relapse mortality (NRM) were analyzed between two groups. Results:The ALC at day 14 after chemotherapy in complete remission group was higher than the no remission group, but the differences were no significant ( P 〉 0.05 ). The DFS of the high ALC group were higher than the low ALC group ( P = 0.009), and the relapse rate were lower than the low ALC group (P = 0. 032). Conclusion:The ALC at day 14 after induction chemotherapy correlated with the prognosis. The detection of lymphocytes had clinical value in evaluating the treat- ment effects and prognosis in AML patients.
