高压氧治疗高原地区乳腺癌术后放疗不良反应的疗效

摘　　要：目的:探讨高压氧疗对高海拔地区乳腺癌患者保乳术后放疗不良反应的疗效。方法:44例女性乳腺癌保乳手术后行局部放疗后皮肤红斑、色素沉着、水肿、疼痛等不良反应的患者,其中32例给予每周5次的高压氧治疗,每次以240 k Pa的压力吸入纯氧90分钟;12例拒绝高压氧疗者作为对照组。接受高压氧治疗的患者采用LENT-SOMA标准进行治疗前、后评估,并针对每个患者制定个体化治疗护理措施。结果:接受个体化高压氧治疗护理的32例患者局部皮肤恢复接近正常,疼痛、水肿、红斑评分显著低于对照组(P<0.001),其中7例患者自觉症状彻底消失,仅纤维化和毛细血管扩张近期改变不明显。而对照组12例患者症状持续存在。结论:高压氧对于高原缺氧地区乳腺癌保乳术后放疗的不良反应具有明显缓解作用。Objective：To investigate the efficacy of hyperbaric oxygen therapy for patients with breast cancer underwent surgery and radiation following adverse reaction in plateau area.Methods：The data of 44 female patients underwent breast conservative surgery and post-operative radiation persisting skin with erythema,edema and severe pain were divided into two groups.32 cases received hyperbaric oxygen therapy in a multiplace chamber for a median of 25 sessions （range 7~60） as a treatment group.One hundred percent oxygen was delivered at 240 kPa for 90 min sessions,5 times per week.Evaluation of the irradiated breast tissue were scored before and after hyperbaric oxygen therapy by using modified LENT-SOMA criteria.Another 12 patients refused to the treatment as a control.Results：The results showed a significant reduction of pain,edema and erythema scores in the treatment group compared with control （P〈0.001）.7 of 32 patients were free of symptoms after hyperbaric oxygen therapy.However,the all 12 patients in the control group were still suffering from the complaints.Conclusion：Hyperbaric oxygen therapy is helpful and benefited to recover in patients persisting skin with erythema,edema and severe pain following breast-conserving therapy and postoperative radiation.
