三维适形放疗联合唑来膦酸治疗骨转移癌的临床效果分析

摘　　要：目的:探讨三维适形放疗联合唑来膦酸对骨转移癌的治疗效果。方法:56例骨转移癌疼痛患者采用三维适形放疗联合唑来膦酸治疗,109例骨转移癌疼痛患者采用普通放疗联合唑来膦酸治疗,分析两组的疼痛缓解总有效率、疼痛缓解起效时间、疼痛缓解维持时间、生活质量改善率、不良反应发生率和疼痛复发率。结果:三维适形放疗联合唑来膦酸治疗组和普通放疗联合唑来膦酸治疗组的疼痛缓解总有效率分别为91.07%、83.49%;疼痛缓解起效时间分别为治疗开始后(7.34±2.54)、(8.13±2.70)d;疼痛缓解维持时间分别为治疗结束后(8.39±4.67)、(6.89±3.90)个月;生活质量改善率分别为96.43%、89.91%;不良反应发生率分别为26.79%、43.12%;疼痛复发率分别为13.64%、31.51%。两组在疼痛缓解总有效率、疼痛缓解起效时间、疼痛缓解维持时间、生活质量改善率比较差异均无统计学意义(P>0.05),但不良反应发生率、疼痛复发率比较差异均有统计学意义(P<0.05)。结论:三维适形放疗联合唑来膦酸治疗骨转移癌效果肯定,与普通放疗联合唑来膦酸治疗相比,不良反应发生率、疼痛复发率低。Objective：To assess the efficacy of three-dimensional conformal radiotherapy combined with Zoledronic Acid in the treatment of bone metastases.Method：56 patients with bone metastases were treated with three-dimensional conformal radiotherapy and Zoledronic Acid.Meanwhile,109 patients with bone metastases were administrated with conventional radiotherapy and Zoledronic Acid.The pain relieve rate,the pain relieve time,the pain relieve lasting time,the life quality improvement rate,the incidence of untoward effect,and the pain recurrence rate were respectively compared between two groups.Result：In the group of three-dimensional conformal radiotherapy combined with Zoledronic Acid,the pain relieve rate,the pain relieve time,the pain relieve lasting time,the life quality improvement rate,the incidence of untoward effect,and the pain recurrence rate were 91.07%,（7.34±2.54）days,（8.39±4.67）months,96.43%,26.79%,and 13.64%.These data were respectively 83.49%,（8.13±2.70）days,（6.89±3.90）months,89.91%,43.12%,and 31.51% in the group of conventional radiotherapy combined with Zoledronic Acid.There were no significant differences between these two groups for the pain relieve rate,the pain relieve time,the pain relieve lasting time,and the life quality improvement rate（P〉0.05）.However,the differences were statistically significant for the incidence of untoward effect and the pain recurrence rate between the two groups（P〈0.05）.Conclusion：Three-dimensional conformal radiotherapy combined with Zoledronic Acid is effective in the treatment of bone metastases.The incidence of untoward effect as well as the pain recurrence rate are lower,compared with conventional radiotherapy combined with Zoledronic Acid.
