摘 要:目的探讨人工耳蜗植入术前MRI内耳水成像的诊断价值。方法回顾性分析136例(272耳)拟行人工耳蜗植入术的重度感音性耳聋患者术前MRI水成像图像资料,结合内耳畸形最新分类标准进行诊断及分类。结果 MRI资料显示272内耳中正常183耳,异常89耳,其中耳蜗异常22耳;前庭异常30耳;半规管异常71耳;内淋巴管及内淋巴囊扩大49耳,内耳道发育异常11耳。结论人工耳蜗植入术前MRI检查作为一种可准确诊断有无内耳畸形的影像学检查,对指导人工耳蜗植入手术方案、预防术中并发症及判断预后有重要价值。Objective To discuss the diagnostic value of inner ear MRI prior to cochlear im- plantation (CI). Methods One hundred and thirty-six cases which clinically diagnosed as binaural severe sensorineural hearing loss (SNHL) were examined by 3.0-T MR scanner prior to CI. According to the new classification for malformations of inner ear structures, a retrospec- tive analysis was made based on the preoperative MRI data of the 136 cases. Results The preoperative MRI data showed that, among the 272 inner ears, 183 inner ears were normal and 89 ears had malformations. For the 89 abnormal inner ears, the MRI results were divided to subgroups as the followings: cochlear, vestibular, semicircular canal (SC) , vestibular aque- duct and internal auditory canal (IAC) malformations. The number of the subgroups is 22, 30, 71, 49 and 11 ears, respectively. Conclusion MRI of the inner ears provides vital infor- mation, it is a important examination in all patients posted for CI. The preoperative MRI pro- vides accurate information and exquisite anatomical detail of the membranous labyrinth and the cranial nerves. It is necessary to guide the implant team in selecting the surgical approach and strategy, preventing of complications and having a significant impact on the prognosis.
