摘 要:目的探讨鱼油脂肪乳剂对脂多糖(LPS)诱导的急性肺损伤(ALI)大鼠肝脏的保护作用及作用机制。方法将60只幼年雄性SD大鼠分为Blank组、LPS组和Omega组,每组20只。穿刺大鼠尾静脉,Blank组和LPS组注射0.9%氯化钠溶液,Omega组注射10%Omegaven,连续7d。第8天向Blank组大鼠气管内喷洒0.9%氯化钠溶液,Omega组和LPS组大鼠喷洒LPS。8h后麻醉大鼠,开腹经髂总静脉抽取血液,并切取肝组织和肺组织,观察各组大鼠肝组织和肺组织的病理改变,检测血清中ALT、AST和前列腺素E_2(PGE_2)浓度。结果 Blank组肝小叶结构完整,肝细胞未见变性;肺泡结构完整,肺泡腔内无明显渗出及炎症细胞浸润。Omega组和LPS组肝组织表现为轻度非特异性炎症改变,肺组织可见明显炎症细胞浸润和出血。LPS组ALT和AST明显高于Blank组,差异均有统计学意义(均P<0.05)。Omega组与Blank组、LPS组ALT和AST比较差异均无统计学意义(均P>0.05)。LPS组PGE_2浓度明显高于Omega组和Blank组,差异均有统计学意义(均P<0.05);但Omega组和Blank组比较差异无统计学意义(P>0.05)。结论鱼油脂肪乳剂能够减轻LPS引起的肝损伤,其可能是通过调节花生四烯酸的代谢对ALI大鼠发挥保护作用。Objective To investigate the hepatoprotective effect and mechanism of fish oil-based lipid emulsion(Omegaven) on rats with lipopolysaccharide(LPS)-induced acute lung injury. Methods Sixty juvenile Sprague Dawley rats were divided into blank group, LPS group and Omegaven group with 20 in each. Normal saline was injected through caudal vein for 7d in blank and LPS groups, while 10% Omegaven was injected in Omegaven group. On the 8th day, rats in blank group received intratracheal administration of normal saline, while those in Omegaven and LPS groups were given LPS. All the animals were sacrificed 8h later, blood samples were taken for ALT, AST and PGE_2 measurement and liver and lung tissues were collected for pathological examinations. Results HE staining showed that the structure of hepatic lobules and alveoli was intact, there were no degeneration in hepatic cells, no exudation and inflammatory cell infiltration in alveoli of blank group, while in Omegaven and LPS groups the liver tissue showed mild non-specific inflammation and the lung tissue presented with significant inflammation cell infiltration and hemorrhage. Serum ALT and AST levels in LPS group were higher than those in blank group(both P〈0.05).Compared with LPS group and blank group, ALT and AST in Omegaven group were not significantly changed(all P〈0.05).Compared with blank and Omegaven groups, PGE_2 levels in LPS group increased significantly(both P〈0.05), while there was no significant difference in PGE_2 level between Omegaven group and blank group(P〈0.05). Conclusion Fish oil-based lipid emulsion(Omegaven) can alleviate liver injury induced by LPS, which may be related to the regulation of arachidonic acid metabolism.
