摘 要:目的观察探究CD40基因型及血清水平与缺血性脑卒中易感性之间的关系。方法选取我院2014年11月-2016月5月收纳的161例缺血性脑卒中患者及159例健康体检患者,使用PCR技术和基因测序法测定CD40基因单核苷酸的rs1883832C/T,rs1569723A/C及rs4810485G/T位点,进行基因分型,采用Elisa法检测血清CD40水平,分析CD40基因位点的等位点作用以及等位基因及基因型频数分析。结果 CD40基因型在缺血性脑卒中组及健康体检组中分布频率符合H-W遗传平衡,具有群体代表性。缺血性脑卒中组及健康体检组CD40基因中rs1883832C/T基因型频率对比,有统计学意义(P<0.05)。缺血性脑卒中组及健康体检组CD40基因中rs1883832C/T基因频率对比,有统计学意义(P<0.05)。对缺血性脑卒中组及健康体检组CD40单倍型基因联合分析发现,缺血性脑卒中组及健康体检组T-C-T单倍型基因分布有明显差异,有统计学意义(P<0.05)。缺血性脑卒中组血清CD40水平显著高于健康体检组(P<0.05)。结论 CD40基因单核苷酸的rs1883832C/T基因型多态性及联合基因型中T-C-T单倍型与缺血性脑卒中病情发生发展密切相关,CD40基因单核苷酸的rs1883832C/T中T等位基因可能是缺血性脑卒中的遗传易感基因,携带T等位基因的人更易出现CD40基因的高度表达,从而诱发缺血性脑卒中发病风险。Objective:To investigate the relationship between CD40 genotypes and serum levels and susceptibility to ischemic stroke. Methods:In our hospital from November 2014-2016 month May received 161 cases of patients with ischemic stroke and 159 healthy patients,determination of CD40 gene single nucleotide PCR and gene sequencing of rs1883832C/T,rs1569723A/C and rs4810485G/T loci,were genotyped using Elisa method to detect the serum level of CD40,the analysis of the CD40 gene locus. Results:The distribution frequency of CD40 genotype in ischemic stroke group and healthy group was in accordance with the H-W genetic equilibrium,and the group was representative. The rs1883832C/T genotype frequencies of CD40 gene in ischemic stroke group and healthy group were statistically significant(P〈 0.05). There was a significant difference in rs1883832C/T gene frequency between CD40 gene and ischemic stroke group(P〈 0.05). Combined analysis of CD40 haplotype gene in ischemic stroke group and health examination group showed that there was significant difference in the distribution of T-C-T haplotypes between the ischemic stroke group and the healthy group(P〈 0.05). The level of serum CD40 in ischemic stroke group was significantly higher than that in healthy group(P〈 0.05). Conclusion:rs1883832C/T gene polymorphism and gene type CD40 gene single nucleotide in T-C-T haplotype and ischemic stroke disease is closely related to the development of T,CD40 gene single nucleotide in the rs1883832C/T gene may be a genetic risk factor for ischemic stroke,T allele were more prone to high expression of CD40 gene thus,induced ischemic stroke risk.
