烟雾病血管搭桥术后颅面软组织缺损游离背阔肌皮瓣修复1例报告

摘　　要：颌面部大面积软组织缺损的修复,需要能提供足够组织量的游离皮瓣。背阔肌皮瓣可达20 cm×15 cm,适用于这类大面积缺损的修复。本文报告1例外伤导致的颅面部大面积软组织缺损病例,颅骨、颧骨、颧弓、硬脑膜暴露,既往有烟雾病病史,曾发生颅内出血,引起神经功能损害及左侧肢体功能障碍,行双侧颅内外血管搭桥手术。该患者面临再次及反复出血的威胁。应用游离背阔肌皮瓣成功修复了颅面部缺损,并避免了颅内严重并发症的发生。Reconstruction of massive maxillofacial soft tissue defect needs free flap such as latissimus dorsi flap which can provide 20cm×15 cm of soft tissue for repair. We reported a woman who presented with massive traumatie soft tissue defect with exposure of the skull, zygoma, and endocranium due to moyamoya disease history. She suffered from intraeranial hemorrhage which caused neural damage and left limb dyskinesia. She underwent bilateral extracranial- intracranial bypass surgery. She was at risk of recurrent hemorrhage.We used free latissimus dorsi flap to reconstruct the defect successfully and avoided severe intracranial complication that might happen.
