摘 要:目的:探讨肝胃百合汤辅助治疗高原消化性溃疡的临床疗效及对患者血清一氧化氮(NO)、转化生长因子-β1(TGF-β1)、Smad4水平的影响。方法:选择2015年5月~2017年5月于上海中医药大学附属龙华医院消化内科接受治疗的82例高原消化性溃疡患者,按治疗方式分作对照组与实验组,对照组选用常规治疗,实验组选用肝胃百合汤治疗。比较两组胃镜及症候疗效,治疗前后症候积分、NO、TGF-β1、Smad4水平的变化,再生粘膜成熟度及并发症的发生情况。结果:治疗后,实验组胃镜及症候总有效率、再生粘膜成熟优良率均显著高于对照组(P<0.05),并发症率明显低于对照组(P<0.05)。治疗后,两组症候积分均较治疗前显著降低,且实验组显著低于对照组;两组NO,TGF-β1,Smad4水平均较治疗前明显上升,且实验组高于对照组,差异均有统计学意义(P<0.05)。结论:肝胃百合汤辅助治疗高原消化性溃疡的临床疗效明显优于常规治疗,可有效促进创面修复并提高治疗安全性,可能与其提高患者的血清NO、TGF-β1、Smad4水平有关。Objective: To observe the influence of ganwei baihe decoction auxiliary on the serum levels of nitric oxide (NO), transforming growth factor-β1 (TGF-β1) and Stand4 of patients with peptic ulcer in plateau. Methods: 82 patients with peptic ulcer in plateau who were treated in our hospital from May 2014 to October 2016 were selected and divided into the control group and the experimental group according to the different treatment methods. Patients in the control group were treated with conventional method, while patients in the experimental group were treated with ganwei baihe decoction. Then the curative effect of gastroscope, the symptom score, the serum levels of NO, TGF-β1 and Smad4, the regenerative mucosal maturity and complications in the two groups were observed and compared before and aRer treatment. Results: The endoscopy total effective rate, symptom total effective rate and regenerated mueosal maturation rate of experimental group were significantly higher than those of the control group (P〈0.05). The incidence of complications in the experimental group was significantty lower than that of the control group (P〈0.05). After treatment, the symptoms score of two groups significantly reduced and the experimental group was lower than that of the control group (P〈0.05); After reatment, the serum levels of NO, TGF-β1 and Smad4 in both groups were significantly increased, which were higher in the experimental group than those of the control group (P〈0.05). Conclusion: Ganwei baihe decoction auxiliary was effective on the peptic deer in in the plateau, which could improve the serum levels of NO, TGF-β1 and Smad4, and enhance the safety.
