摘　　要：目的:探讨脂联素、瘦素和肿瘤坏死因子(TNF)-α与酒精性脂肪肝严重程度的相关性。方法:45只Wistar大鼠分为对照、饮酒和甜菜碱组。酶比色法检测肝脏三酰甘油(TG)含量。酶联免疫吸附测定脂联素、瘦素和TNF-α。相关性采用Pearson相关和多元线性回归分析。结果:饮酒导致肝脏TG含量增高,血清和肝脏脂联素降低、瘦素增高。与饮酒组相比,甜菜碱组肝脏TG减少,血清、肝脏脂联素增高,血清、肝脏瘦素无差异。肝脏TNF-α水平在3组间无差异。血清脂联素与肝脏TG含量呈负相关(r=-0.605,P=0.000)。血清瘦素、肝脏脂联素和瘦素不是肝脏TG含量的独立影响因子。结论:血清脂联素是预测酒精性脂肪肝严重程度的重要指标,提高血清脂联素可降低肝脏TG含量,瘦素和TNF-α与酒精性脂肪肝无独立相关性。Objective To study the association of adiponeetin, leptin and tumor necrosis factor-α（TNF-α） with severity of alcoholic fatty liver. Methods Forty-five Wistar rats were divided into control, ethanol-fed and betaine-fed groups. Hepatic triglyceride was measured by enzymatic eolorimetrie method. Adiponectin, leptin and TNF-α levels were determined by enzyme-linked immunosorbent assay. Pearson correlation analysis and multivariable linear-regression analysis were used to detect the association between these factors and alcoholic fatty liver. Results Ethanol resulted in increased triglyeeride content in liver, reduced adiponeetin and increased leptin in serum and liver. Compared with ethanol group, hetaine reduced hepatic triglyeeride, up-regulated serum and hepatic adiponectin, but did not affect leptin in liver and serum. There was no significant difference in hepatic TNF-c~ among these three groups. Serum adiponectin was negatively correlated with triglyceride content in liver （r=-0.605, P=0.000）. Serum leptin, liver adiponectin and leptin were not the independent influencing factors of triglyceride content in liver. Conclusions Serum adiponeetin was an important predictor of severity of alcoholic fatty fiver, increasing serum adiponectin might reduce the triglyceride content in liver. Leptin and TNF-α did not correlate independently with alcoholic fatty liver.
