摘  要:目的系统评价PPAR-γ 2基因rs1801282 C>G多态性与2型糖尿病肾病(T2DN)遗传易感性。方法应用PubMed、中国知识资源总库(CNKI)和万方数据库,检索2017年3月前发表的rs1801282多态性与T2DN易感性关联研究。应用Stata 12.0对研究数据统计分析,以合并OR值及相应95%置信区间(95%CI)评价rs1801282与T2DN易感性关系。结果根据纳入、排除标准,纳入16篇病例-对照研究,研究对象9 183例。等位基因比较显示,与rs1801282 C相比,G等位基因可显著降低T2DM患者发生DN的风险,合并效应的OR值及95%CI为0.74(0.62~0.89),P=0.002;以种族进行亚组分析,发现G等位基因与高加索人T2DN发病风险降低有关,OR值及95%CI为0.75(0.60~0.94),P=0.012。基因型分析显示,显性遗传模型(CG+GG vs.CC)下,合并效应的OR值及95%CI为0.72(0.59~0.87),P=0.001;亚组分析发现,rs1801282多态位点与高加索人T2DN遗传易感性有关,OR值及95%CI为0.71(0.56~0.91),P=0.006。结论 PPAR-γ 2基因rs1801282位点与T2DN发病有关,G等位基因可能是机体防止DN发生的保护性遗传因素。Objective To systematically investigate the genetic susceptibility between PPAR-γ2 rs1801282 C 〉G polymorphism and T2DN. Methods PubMed, CNKI and Wanfang data base were searched before March, 2017, for eligible studies. Meta-analysis was performed by using Stata 12.0 software, and OR with corresponding 95%CI were adopted to assess the susceptibility of rs1801282 and T2DN. Results A total of 16 case-control studies with 9 183 subjects were included. Meta-analysis showed that rs1801282 G allele could decrease the risk of T2DM suffering from DN, the OR and corresponding 95%CI were 0.74(0.62 - 0.89), P=0.002. Subgroup analysis by ethnic showed that rs1801282 was significantly associated with DN in Caucasians, and the OR and 95% CI were 0.75 (0.60 - 0.94), P=-0.012. Genotype analysis in the dominant model showed that rs1801282 C〉G was significantly associated with T2DN, and the OR and 95%CI were in overall 0.72(0.59 -0.87), P=0.001, and in Caucasians 0.71 (0.56 -0.91), P=0.006. Conclusion PPAR-γ2 rs1801282 polymorphism was closely related to the genetic susceptibility to T2DN, and D allele might a protection genetic factor of decrease the risk of T2DM to DN.
