强化改良CHOP方案治疗非霍奇金淋巴瘤疗程及近远期疗效分析

摘　　要：目的探讨强化改良CHOP方案治疗非霍奇金淋巴瘤(NHL)的疗程数、近远期疗效分析。方法选取2014年2月—2015年6月河南省肿瘤医院中西医肿瘤内科收治的80例NHL患者,随机分为观察组和对照组各40例。对照组给予常规CHOP方案化疗,观察组在对照组基础上另强化改良化疗方案治疗。观察化疗后两组疗程数、远近期疗效及用药安全性比较。结果观察组的总有效率(35.00%)较对照组(12.50%)显著提高,差异有统计学意义(P<0.05);观察组患者疗效达到完全缓解、部分缓解所需疗程数[(5.13±1.12)、(4.10±1.02)个]分别均较对照组[(8.28±1.62)、(6.10±1.03)个]显著缩短(t=10.116,P<0.05;t=8.726,P<0.05);两组远期生存率及不良反应发生率均无统计学意义(P>0.05)。结论强化改良化疗方案治疗NHL临床总有效率较高,可显著减少达到临床疗效所需的疗程数,降低复发率,远期生存率方面有待做进一步大样本量研究。Objective To analyze the treatment courses,short and long term curative effects of intensive modified CHOP in the treatment of non Hodgkin＇s lymphoma（ NHL）. Methods A total of 80 NHL patients treated in our hospital from Feb. 2014 to Jun. 2015 were randomly divided into the observation group and control group. Both groups were given conventional CHOP chemotherapy,besides,the observation group was given additional intensive modified chemotherapy regimen. The treatment courses,short and long term curative effects,and medication safety in the two groups were observed and compared. Results The total effective rate in the observation group was 35. 00% which was significantly higher than that in the control group 12. 50%（ P〈0. 05）; The complete and partial remission courses in the observation group were（ 5. 13 ± 1. 12） and（ 4. 10 ± 1. 02） which were significantly lower than those in the control group（ 8. 28 ±1. 62） and（ 6. 10 ± 1. 03）（ t = 10. 116,P〈0. 05; t = 8. 726,P〈0. 05）; There were no significant differences in long-term survival rate and incidence rate of adverse reactions between the two groups（ P〈0. 05）. Conclusion Intensive modified chemotherapy regimen which has higher total effective rate in clinic,can significantly reduce treatment courses and recurrence rate for NHL patients,and its long term survival rate needs further study on large sample.
