吞咽康复训练联合中药冷热刺激法治疗痰瘀互结型脑卒中吞咽障碍效果观察

摘　　要：目的:观察吞咽康复训练联合中药冷热刺激法治疗痰瘀互结型脑卒中后吞咽障碍患者的临床效果。方法:纳入72例脑卒中吞咽障碍患者,随机分为对照组和观察组各36例,对照组给予吞咽康复训练,观察组以吞咽康复训练联合中药冷热刺激法治疗。2组均以1周为1疗程,治疗4周。观察2组的临床疗效和治疗2周、4周的美国国立卫生研究院脑卒中量表(NIHSS)评分、Burke吞咽障碍筛查量表评分。结果:观察组总有效率94.44%,对照组总有效率75.00%,2组比较,差异有统计学意义(P<0.05)。治疗2周、4周,2组NIHSS评分、Burke评分均较治疗前降低(P<0.05,P<0.01),观察组2项评分均低于对照组(P<0.05)。结论:吞咽康复训练联合中药冷热刺激法能提高脑卒中吞咽障碍患者的吞咽功能,且能防止舌肌、咽肌群废用性萎缩,增强局部肌肉的协调性,恢复、重建吞咽功能。Objective： To observe the clinical effect of swallowing rehabilitation training combined with hot and cold stimulation with Chinese medicine for stroke and dysphagia with phlegmand blood stasis type. Methods：Divided 72 cases of patients with stroke and dysphagia into the control group and the observation group randomly,36 cases in each group. The control group was treated with swallowing rehabilitation training,while the observation group was treated with swallowing rehabilitation training combined with hot and cold stimulation with Chinese medicine. Both groups received treatment for one week as a course,four weeks in total. Observed the clinical effect,score of national institute of health stroke scale（NIHSS）and score of Burke dysphagia screening scale after two and four weeks of treatment in both groups. Results： The total effective rate was 94.44% in the observation group and 75.00% in the control group,the difference being significant（P0.05）.After two and four weeks of treatment,scores of NIHSS and Burke in both groups were decreased when compared with those before treatment（P0.05,P0.01）,and the two scores in the observation group were lower than those in the control group（P0.05）. Conclusion：The therapy of swallowing rehabilitation training combined with hot and cold stimulation with Chinese medicine can enhance the swallowing function of patients with stroke and dysphagia. It can also prevent disuse atrophy of tongue muscles and pharyngeal muscles,enhance the coordination of local muscles,and restore and reconstruct the swallowing function.
