CTA点征“九分法”联合FIB表达水平评估高血压脑出血预后的效果

摘　　要：目的探讨CTA点征"九分法"联合FIB表达水平评估高血压脑出血的科学性。方法回顾性收集高血压脑出血患者,根据"九分法"分为0~3分组42例和4~9分组56例,比较两组临床资料、点征阳性、血肿扩大情况、30 d NIHSS评分结果、外科手术干预及两组FIB表达水平,并对两组进行Kaplan-Meier生存分析。结果与0~3分组比较,4~9分组入院血肿量、点征出现比例明显升高,差异具有统计学意义(P<0.05);与0~3分组比较,4~9分组点征阳性,血肿扩大、30 d NIHSS评分及外科手术干预明显较高,差异具有统计学意义(P<0.05);与0~3分组比较,4~9分组在3 d前FIB明显较高,差异具有统计学意义(P<0.05);两组患者的中位生存时间分别为66.00、22.00个月,差异有统计学意义(P<0.05)。结论 CTA点征"九分法"联合血液FIB表达水平能更好地预测高血压脑出血的预后,为患者提供切实的治疗方案,改善患者生活质量。Objective To investigate the scientificity of CTA spot sign ＂nine points method＂ combined with FIB expression level in the evaluation of hypertensive intracerebral hemorrhage. Methods The patients with hypertensive intracerebral hemorrhage were retrospectively collected. The patients were divided into 0-3 points group of 42 cases and 4-9points group of 56 cases according to the ＂nine points method＂ after admission. The clinical data of disease, positive spot sign, hematoma enlargement, 30 d NIHSS score, surgical intervention and FIB expression level were compared between the two groups. Kaplan-Meier survival analysis was carried out in both groups. Results Compared with 0-3points group, the volume of hematoma and the proportion of appearance of dot sign were significantly higher in 4-9points group. The difference was statistically significant（P〈0.05）. Compared with 0-3 points group, the positive dot sign,hematoma enlargement, 30 d NIHSS score and surgical intervention in the 4-9 points group were significantly higher,and the differences were statistically significant（P〈0.05）; Compared with 0-3 points group, FIB was significantly higher in 4-9 points groups before 3 days, and the difference was statistically significant（P〈0.05）; The median survival time of the patients in both groups was 66.00 and 22.00, and the difference was statistically significant（P〈0.05）. Conclusion CAT spot sign ＂nine points method＂ combined with blood FIB expression level can better predict the prognosis of hy pertensive intracerebral hemorrhage, provide a practical treatment scheme for the patients and improve their quality of life.
