丁苯酞软胶囊、依达拉奉联合治疗急性脑梗死的临床效果观察

摘　　要：目的探讨丁苯酞软胶囊、依达拉奉联合治疗急性脑梗死的临床效果观察。方法将2016年4月~2017年4月在我院神经内科治疗的124例急性脑梗死患者随机分为两组,两组均给予常规对症支持治疗,在此基础上,对照组给予依达拉奉注射液静脉滴注30 mg/次,2次/d,观察组在对照组基础上给予丁苯酞软胶囊0.2 g/次,3次/d,比较两组患者的临床疗效、血流动力学变化、神经功能及日常生活能力变化、各项细胞因子水平变化。结果观察组临床总有效率为93.55%,较对照组的79.03%明显上升,两组比较差异有统计学意义(P<0.05);观察组治疗后全血高切黏度、全血低切黏度、血浆黏度、纤维蛋白原、血细胞比容较对照组明显降低,两组比较差异有统计学意义(P<0.05);观察组治疗后NIHSS评分及ADL评分较对照组明显改善,两组比较差异有统计学意义(P<0.05);观察组治疗后TNF-α、IL-1、SOD、MDA、NT-pro BNP较对照组明显改善,两组比较差异有统计学意义(P<0.05)。结论丁苯酞软胶囊、依达拉奉联合治疗急性脑梗死具有良好临床效果,能切实改善脑血流动力学,发挥脑保护作用,从而改善神经功能,具有积极的临床意义。Objective To investigate the clinical effect of butylphthalide soft capsule combined with edaravone in the treatment of acute cerebral infarction. Methods A total of 124 patients with acute cerebral infarction who were admitted to the Department of Neurology in our hospital from April 2016 to April 2017 were random Ly assigned to two groups.Both groups were given routine symptomatic supportive therapy. On this basis, the control group was given intravenous infusion of edaravone injection, with 30 mg per injection twice a day. The observation group was further given butylphthalide soft capsules, with 0.2 g for 3 times a day on the basis of the control group. The clinical efficacy, hemodynamic changes, changes of neurological function and daily life ability, and changes of levels of cytokines were compared between the two groups of patients. Results The clinical total effective rate was 93.55% in the observation group, which was significantly higher than that in the control group（79.03%）, and there was significant difference between the two groups（P〈0.05）. The high blood viscosity, low blood viscosity, plasma viscosity, fibrinogen and hematocrit in the observation group after the treatment were significantly lower than those in the control group, and the differences were statistically significant（P〈0.05）; the NIHSS score and ADL score in the observation group after the treatment were significantly improved compared with the control group, and there were statistical differences between the two groups（P〈0.05）.The improvement of TNF-α, IL-1, SOD, MDA and NT-pro BNP after the treatment was significantly better in the observation group than the control group, and the differences were statistically significant（P〈0.05）.
