羊膜移植和唇粘膜移植治疗重度义眼座暴露的疗效比较

摘　　要：目的比较羊膜移植和唇粘膜移植在治疗重度义眼座暴露中的疗效。方法选取重度义眼座暴露患者12例12眼,随机分为2组,每组6例,分别用羊膜移植和唇粘膜移植来进行义眼座暴露修复手术,观察2组病例手术疗效并进行比较。结果羊膜移植组6例中有2例结膜能沿羊膜移行生长至完全遮盖义眼座,达到义眼座无暴露、无感染、结膜囊无狭窄的治愈标准,其余4例分别在术后1-3周内羊膜脱落,手术失败。而唇粘膜移植组6例移植的唇粘膜全部存活,均达到治愈标准。结论唇粘膜移植治疗重度义眼座暴露,组织新鲜,血运丰富,易存活,效果明显优于羊膜移植。Objective To comparison the clinical efcacy of amniotic membrane transplantation and lip mucosa transplantation in the treatment of severe ocular prosthesis exposure .Methods 12 cases （12 eyes） with severe ocular prosthesis （HA） exposed were randomly divided into two groups, each group has 6 cases, to make the reconstructive surgery respectively with amniotic membrane transplantation and lip mucosa transplantation.Then we observed and compared the two groups cases surgical curative efect.Results In the amniotic membrane transplantation group, 2 cases of conjunctival can grow along the amniotic membrane migration to completely cover the ocular prosthesis ,can live up to the cured standards of ocular prosthesis without exposure、infection and conjunctival sac without narrow . In the rest 4 cases , amniotic membrane has been falled of respectively in 1-3 weeks after surgery, the operation is failed. And in the 6 cases of lip mucosa transplantation group ,lip mucosa all survived, achieves cure standard.Conclusion It was recognised that in the treatment of severe ocular prosthesis exposure, Lip mucosa transplantation was fresh, rich blood supply and easy to survive, the efect is obviously better than that of amniotic membrane transplantation.
