宫颈锥切术对宫颈上皮内瘤变Ⅲ级患者CDK8和KAI1蛋白的影响

摘　　要：目的探讨研究宫颈锥切术对宫颈上皮内瘤变Ⅲ级患者周期素依赖性激酶8(CDK8)、KAI1蛋白的影响。方法回顾性分析2012年4月到2013年10月收治的宫颈上皮内瘤变Ⅲ级患者,其中行宫腔镜下宫颈锥切术45例(A组),行全子宫切除术32例(B组)。对两组患者手术时间、出血量以及手术前后CDK8(OD值)、KAI1蛋白(阳性率)变化,术后12个月患者恢复情况进行分析。结果 A组患者手术时间和术中出血量明显低于B组患者(P<0.05);术前两组患者CDK8蛋白表达和KAI1蛋白比较无显著差异,而手术后两组CDK8蛋白表达(OD值)显著下降,KAI1蛋白阳性率比手术前显著升高(P<0.05),而两组比较无显著差异(P>0.05);术后随访12个月后,A组治疗无效率(57.78%)明显高于B组(34.38%),差异具有显著性(P<0.05)。结论宫腔镜下宫颈锥切和筋膜外子宫全切效果相当,均可以抑制CDK8蛋白的表达,提高KAI1蛋白表达活性。但是二者各有优缺,具体治疗方式选择需要各方面综合考虑。Objective To discuss the effect of conization on CDK8,KAI1 protein in patients with cervical intraepithelial neoplasia gradeⅢ. Methods Patients with cervical intraepithelial neoplasia grade Ⅲ treated in our hospital from April 2012 to October 2013 were retrospectively analyzed,including 45 cases of hysteroscopic conization（ A group）,32 cases of total hysterectomy（ group B）. The operation time,blood loss and CDK8（ OD value）,KAI1 protein（ positive rate） changes before and after surgery,recovery after 12 months of the two groups were analyzed. Results Operative time and blood loss in group A were significantly lower than those in group B（ P〈0. 05）. The differences of CDK8 protein and KAI1 protein before surgery of the two groups were not statistically significant,but CDK8 protein（ OD） of the two groups after surgery decreased significantly. The positive rate of KAI1 protein was significantly higher than that before surgery（ P〈0. 05）,and there was no difference between the two groups（ P〈0. 05）. Postoperative follow-up of 12 months,the inefficiency rate of group A was 57. 78%,and group B 34. 38%,and the difference was statistically significant（ P〈0. 05）. Conclusion Hysteroscopic conization and hysterectomy fascia can both inhibit the expression of CDK8 protein and improve KAI1 protein activity. But they both have their own advantages and disadvantages,so specific treatment preferences must be considered in all aspects.
