脑室内中枢神经细胞瘤的MR分析与病理对照

摘　　要：目的分析中枢神经细胞瘤的MR特点及病理表现,提高对本病的MR认识。方法回顾性分析经病理证实的11例中枢神经细胞瘤的MR表现并与病理对照分析。结果右侧脑室体部前2/3区域7例,右侧脑室体部后2/3区域2例,Monro孔区域2例。MR表现为T1WI呈等低混杂信号,T2WI呈高信号,病灶信号多不均匀,72.7%(8/11)呈典型的"蜂窝征"改变,并见"血管瘤空征",81.8%(9/11)见"扇形征"或"绳索征"。2例合并出血,1例合并海绵状血管瘤。增强9例实性部分不均匀中度强化,1例显著强化,1例未强化。11例免疫组化染色突触蛋白(Syn)、神经元特异性烯醇化酶(NSE)神经元标记物均呈阳性。结论中枢神经细胞瘤的MR有一定特征性表现,加深认识,可提高诊断准确率。Objective To investigate the MRI features of the central neurocytoma and to improve preoperative diagnosis. Methods MR imagings of 11 patients with central neurocytoma confirmed surgically and pathologically were retrospectively re-viewed and compared with pathological analysis. Results 7 cases located in the anterior 2/3 of the right of lateral ventricle, 2 cases located in the posterior 2/3 of the lateral ventricle, 1 case located the left of lateral ventricle and 2 cases located in the fora-men of monro. Most tumors presented as isointensity or slight hypointensity signal on T1 WI and isointensity or slight hyperintensity signal on T2 WI. MRI manifestations within the solid tumor included＂honeycomb＂or＂vegetablesponge＂and flow void effect （72. 7%, 8/11）, scalloping appearance （81. 8%, 9/11）. 2 cases presented with bleeding, 1 case with cavernous hemangioma. On enhanced MRI, 9 cases of the lessions had slight enhancement, 1 case had marked inhomogeneous enhancement and 1 case had no enhancement. Immunohistochemistry examination was employed in 11 cases. Syn and NSE were positive. Conclusion The MR imagings of CNC have the better characteristic appearance, and to deepen understanding can improve the diagnostic accu-racy.
