小切口非超声乳化白内障摘出术治疗硬核白内障的效果

摘　　要：目的评价小切口非超声乳化白内障摘出人工晶状体植术治疗硬核白内障的效果。方法反眉形小切口手法碎核，摘出Ⅳ～V级硬核白内障并植入人工晶状体198例（198眼）。结果术后1周矫正视力≥0．5者占67．17％，≥1．0者占36．36％；术后3个月矫正视力≥0．5者占93．43％，≥1．0者占75．76％。术后1周散光为（1．50±0．75）D，3个月散光为（0．75±0．50）D。术后并发症：轻度角膜水肿19眼，占9．60％，虹膜炎13眼，占6．57％，后囊轻度浑浊14眼，占7．07％，无严重并发症发生。结论非超声乳化小切口白内障摘出术具有散光度低、并发症少、视力恢复快的特点，同时避免了超声能量引起的角膜损伤等严重并发症。Objective To evaluate the efficacy of small incision non-phacoemulsification cataract extraction and intraocular lens implantation for the treatment of hard nucleus cataract. Methods The data of 198 eyes of 198 cases with hardness nucleus grades 1V-V were collected and they received small incision cataract extraction by manual nuclear fragmentation and implantation of intraocular lens. Results The eyes with best corrected visual acuity（BCVA） ≥0.5 accounted for 67.17% and BCVA ≥ 1.0 for 36.36% 1 week after surgery. Three months after surgery the eyes of BCVA/〉 O. 5 were in 93.43% and ≥ 1.0 in 75.76% . The astigmatism was （ 1.50 ± 0.75 ） D 1 week after surgery,and（0.75 ± 0.50） D 3 months after surgery . Postoperative complications were mild corneal edema in 19 eyes （9. 60% ）, iritis in 13 eyes （6.57%） ,and posterior capsule mild turbidity in 14 eyes （7.07%）. No serious complication occurred. Conclusion Small incision non-phacoemulsification cataract surgery is effective with low astigmatism, less complications and fast vision recovery . It avoides corneal injury caused by the ultrasonic energy.
