Her-2、Ki-67在胃癌中的表达及其临床意义

摘　　要：目的检测Her-2、Ki-67在胃癌中的表达及与临床病理的相关性。方法检测55例胃癌术后标本中Her-2、Ki-67的表达,结合病理及随访综合分析。结果全组Her-2表达阳性者24例(43.6%),Ki-67表达阳性40例(72.7%),经χ~2检验,两组阳性表达在不同病理类型中无差别(P>0.05)。Her-2的阳性表达,在浸润的深度、淋巴结转移和临床分期中,经χ~2检验,均有差异性(P<0.05)。Ki-67表达在后两者中有明显差别(P<0.05)。全组20例(36.4%)同时表达Her-2和Ki67,经Spearman分析,有高度相关性(P<0.05)。Her-2表达阳性的无病生存期及中位生存期,经Kaplan-Meier分析均低于表达阴性者(P<0.05)。结论 Her-2、Ki-67表达与胃癌生物学行为密切相关,对胃癌预后评估及其治疗的选择有一定指导意义。Objective To examine the expression of Her-2 and Ki-67 in different gastric carcinoma and their correlation with clinicopathological features. Methods The expression of Her-2 and Ki-67 in 55 cases of gastric carcinoma were examined. Comprehensive analysis was carried out combining with the clinicopathological features and the data from following up of the patients. Results The positive expression rates of Her-2 and Ki-67 were 43. 6% and 72. 7% respectively and no difference was observed between various types of gastric carcinoma tissues （P 〉0. 05）. The positive expression of Her-2 was correlated with depth of invasion,lymph node metastasis and TNM stage （P 〈0. 01）. There was significant difference in the expression of Ki-67 in lymph node metastasis and TNM stage （P 〈0. 05）. 20 of all patients （36. 4%） expressed both Her-2 and Ki-67 and through Spearman analysis,which showed high relativity （P 〈0. 05）. Her-2 positive patients had significantly lower survival time （including disease-free survival time and the median survival time）than Her-2 negative patients through Kaplan-Meier analysis （P 〈0. 05）. Conclusion The expression of Her-2 and Ki-67 are closely related to the biological behaviors of gastric cancer and is helpful in evaluating the prognosis of patients with gastric cancer and choosing the treatment methods.
