摘  要:目的系统评价氨基酸肠外营养支持对肺癌化疗患者免疫功能的影响;方法通过检索中国生物医学文献数据库、万方数据库、CNKI、维普数据库,搜素时间为各库建库时间至2016年5月,中文主要检索词:氨基酸、肺癌、肠外营养、免疫功能、化疗等,英文主要检索词:Amino acids、lung cancer、Parenteral nutrition、immune function、chemotherapy等。搜索肺癌患者化疗期间对比单纯化疗与氨基酸(9-复合氨基酸)肠外营养支持的临床随机对照试验文献,不限文种,由2位评价员按纳入与排除标准独立筛选文献,提取资料并评价偏倚风险,采用Rev.Man5.2进行Meta分析;结果最终纳入5个随机对照实验(RCT),文献质量均为B级,总共包括506例患者,Meta分析结果显示,观察组患者化疗一个周期内各项免疫指标均优于对照组,其MD和95%CI分别为:CD3[1.98(1.40~2.55)]、CD4[4.47(3.73~5.21)]、CD8[1.08(0.32~1.85)]、CD4/CD8[0.16(0.09~0.28)]、IgG[2.24(1.77~2.70)]、IgA[0.39(0.27~0.51)]、IgM[0.21(0.14~0.28)],与对照组有显著差异(P<0.01)。结论氨基酸肠外营养支持可以改善肺癌患者化疗期间的免疫功能。Objective To systematically review the efficacy of amino acids in parenteral nutrition on immune function in chemotherapy in lung cancer patients. Methods The following electronic databases as CBM, WangFang Date,CNKI and VIP were searchedfrom database inception to May 2016. collect randomized controlled trials (RCTs) of amino acid (9AA) in parenteral nutrition combined with chemotherapy in any language were included. The data were collected by two reviewers, and the data were extracted and evaluated for biased risk. Rev.Man5.3 was used for meta-analysis. Results Five RCT were eventually included, including 506 patients. The results of recta-analysis showed that, the immune indexes of observation group was significantly better than the control group. MD and 95%CI: CD3[1.98(1.40-2.55)], CD414.47(3.73 -5.21 )], CD8[1.08(0.32 - 1.85)], CD4/CD8[0.16(0.09 -0.28)], IgG[2.24(1.77-2.70)], IgA[0.39(0.27 -0.5 1)], IgM[0.21(0.14 -0.28)], (P〈0.01). Conclusion Amino acids in parenteral nutrition shows amarked positive effect on theimmune function of chemotherapy in lung cancer patients.
